摘  要:目的探讨血浆载脂蛋白E(apolipoprotein E,ApoE)基因多态性和脑出血的相关性。方法检测189例原发性高血压脑出血患者及61例健康对照ApoE基因多态性,患者依脑出血部位(脑叶、基底节、脑干、小脑)进行临床分型,比较ApoE基因型和等位基因频率与脑出血及其临床分型之间的关系;比较脑出血患者含有ε4等位基因型组与非ε4等位基因型组入院时GCS(Glasgow coma scale)评分及入院1月后GOS(Glasgow outcome score)评分。结果 (1)脑叶出血组等位基因ε4频率显著高于对照组(P<0.05),其余组与对照组间差异无统计学意义(P>0.05)。(2)含有ε4等位基因型脑出血组较非ε4等位基因型组脑出血入院时GCS无显著差异(P>0.05),但入院1月后含有ε4等位基因型脑出血组GOS评分预后不良明显高于非ε4等位基因型组(P<0.05)。结论 ApoEε4等位基因增加了脑叶出血的风险,含有ε4等位基因型的脑出血患者预后不佳。Objective To explore the correlation between the plasma apolipoprotein E (ApoE)gene polymorphism and the cerebral hemorrhage. Methods Plasma apolipoprotein E gene polymorphism was detected in 189 hypertensive patients with cerebral hemor- rhage and 61 healthy controls. The relationships of ApoE genotype and allele frequency with the cerebral hemorrhage were analyzed. The Glasgow coma scale(GCS) score at the admission and Glasgow outcome score(GOS)score at 1 month after admission were compared a- mong the cerebral hemorrhage patients with epsilon 4 (ε4) alleles and without the epsilon 4 alleles. Results ① The frequencies of epsilon4 alleles in cerebral lobes hemorrhage group were significantly higher than that in normal control group( P 〈 0.05 ). ② There was no significant difference in GCS between cerebral hemorrhage patients with epsilon 4 (ε4)alleles and without the epsilon 4 alleles at the admission(P 〉 0.05). The rate of defective prognosis in the cerebral hemorrhage with ε4 was significantly higher than that of patients without ε4 at 1 month after the admission (P 〈 0.05). Conclusion The subjects carrying ApoE epsilon4 alleles is liable for incidence of cerebral lobe hemorrhage, and the cerebral hemorrhage patients with e4 may have poor prognosis.
