摘  要:目的检测唐山地区甲状腺疾病患者的尿碘水平,分析尿碘水平与甲状腺疾病之间的相关性。方法收集本市工人医院正常成人体检者130例尿样作为对照组,同时收集5种甲状腺疾病患者尿样共285例,按病种分为5组。对所得各组的尿碘检测数据进行多组比较和两两比较。结果各甲状腺疾病组与对照组的多组比较差异有统计学意义(P<0.05)。两两比较表明,桥本氏甲状腺炎和甲状腺功能减退者与对照组比较,差异有统计学意义(P<0.05)。而甲状腺癌、甲状腺结节、毒性弥漫性甲状腺肿者的尿碘水平与对照组相比差异无统计学意义(P>0.05)。各甲状腺疾病组中均有较高比例的高碘和低碘情况存在。结论碘摄入过多可能与桥本氏甲状腺炎及甲状腺功能减退的发病有关。碘在甲状腺癌、甲状腺结节、毒性弥漫性甲状腺肿发病机制中的作用有待进一步研究。应依据个体化尿碘检测结果而不是依据病种来指导碘的摄入。Objective To analyze the relationship between thyroid disease patients and their urinary iodine levels in Tangshan area by detecting the concentration of urinary iodine.Methods 130 urine samples were collected from normal adults for physical examination in Tangshan Worker's Hospital; at the same time,a total of 285 urine samples were collected from patients with5 kinds of thyroid diseases,which were divided into 5 groups.All the urinary iodine detection data were analyzed in pairwise comparison and multiple comparison.Results There was statistical significance on the difference between thyroid diseases group and control group(P〈0.05).Hashimoto's thyroiditis and hypothyroidism were compared with the control group by pairwise comparison,with the differences statistically significant(P〈0.05).The urine iodine levels of thyroid carcinoma,thyroid nodules and greves disease were compared with the control group by pairwise comparison,with the differences not statistically significant(P〉0.05).There was a high proportion of iodine excess and iodine deficiency in all thyroid diseases.Conclusion Excessive iodine intake may be related with hashimoto's thyroiditis and hypothyroidism.The role of iodine in the pathogenesis of thyroid carcinoma,thyroid nodules and greves disease needs to be further studied.The guidance of intake of iodine should be on the basis of individual urine detection results rather than on the basis of disease.
