摘 要:目的研究立体定向血肿碎吸术对大鼠脑出血后血肿周围脑组织水通道蛋白4(AQP4)表达的影响。方法成年雄性Wistar大鼠135只,随机分为假手术组、对照组与治疗组,每组各45只大鼠。假手术组应用立体定位仪向大鼠脑基底节部注入生理盐水;对照组向大鼠脑基底节部注入胶原酶造成脑出血不抽吸;治疗组模型向大鼠脑基底节部注入胶原酶造成脑出血,12h后进行血肿碎吸。采用Berderson评分法评价各组大鼠神经功能缺损程度,采用免疫组化法和Western blot方法检测血肿周围脑组织AQP4蛋白的表达水平。结果 24h时,对照组神经功能缺损、AQP4蛋白的表达量与假手术组比较明显增高(t=10.29、20.92,P<0.05)。同24、120h比较,72h时对照组大鼠的神经功能缺损程度及AQP4表达达到高峰(t=4.54~5.91,P<0.05)。脑出血后72、120h时,治疗组大鼠神经功能缺损程度、AQP4表达与对照组比较明显降低(t=7.56~11.74,P<0.05)。结论脑出血后脑水肿形成可能与AQP4表达有关,而血肿碎吸术可能通过抑制AQP4的表达减轻脑出血后脑水肿的程度。Objective To observe the effect of stereotactic hematoma aspiration on the expression of aquaporin-4(AQP4)in perihematomal tissue after intracerebral hemorrhage in rats. Methods One hundred and thirty-five male adult Wistar rats were evenly randomized to three groups as sham operation group,control group and treatment group.Using stereotaxic instrument,rats in the sham operation group were injected with normal saline into basal ganglia;those in the control group were injected with collagenase into basal ganglia to form an intracerebral hematoma with no suction,and those in the treatment group with collagenase to cause an intracerebral hematoma,which was aspirated after 12 h.Adopting Berderson's test,the degree of nervous function deficiency of rats in each group was assessed.Employing immunohistochemistry and Western blot,the expression of AQP4 in brain tissue around hematoma was detected. Results At 24 h,the degree of nervous function deficiency and AQP4 expression in the control group were higher versus that in the sham operation group(t=10.29,20.92;P〈0.05).Compared with at 24h and 120 h time points,the degree of nervous function deficiency and the expression of AQP4 at 72h time point,of the control group,reached to peak(t=4.54-5.91,P〈0.05).At 72 hand 120htime point after cerebral hemorrhage,the degree of nervous function deficiency and the expression of AQP4 in tissue around hematoma were lower than that in the control group(t=7.56-11.74,P〈0.05). Conclusion Formation of brain edema after intracerebral hemorrhage may be related to the expression of AQP4,and hematoma aspiration to reduce the degree of brain edema after intracerebral hemorrhage is likely through inhibition of AQP4 expression.
