摘  要:目的 探讨高度近视眼晶状体厚度及位置的特点及影响因素。方法 临床病例回顾性研究。对2012年1月至2014年1月于武汉爱尔眼科医院就诊的高度近视患者532例(986只眼)。小瞳检影验光确定等效球镜(SE),Pentacam测量前房深度(ACD),眼部A超测量晶状体厚度(LT)、眼轴长度(AL),计算眼前节长度(ASL)、晶状体位置(LP)、晶状体相对位置(RLP)。分析晶状体厚度与各因素的相关性,按年龄、眼轴分组比较不同组晶状体相关参数的变化。结果 平均年龄为(26.34±6.59)岁,平均SE为(-11.93±3.74)D,平均LT为(3.82±0.29)mm。随着年龄增长,LT增加,ASL增长,LP后移,RLP前移(均P〈0.05),ACD变化无统计学意义(P=0.054)。随着AL增长,LT增加,ACD加深,ASL增长,LP后移,RLP前移(均P〈0.05)。高度近视眼LT与年龄、AL呈正相关,与ACD呈负相关(P=0.000),其中与年龄的相关系数最大(偏相关系数r=0.551,P=0.000),随年龄增长LT增加约0.025 mm/y。结论 对于高度及超度近视患者而言,晶状体随年龄、眼轴的增加逐渐增厚、晶状体中心及后表面均后移,但因眼轴增长,晶状体相对位置前移。Objective To explore the characteristics and influence factors of high myopia lens thickness and location.Methods SE, ACD, LT and AL value were measured on 986 eyes. And ASL, LP, RLP were calculated. The subjects were grouped based on age and AL, and the lens parameters were compared and analyzed among the groups.Results The mean age was 26.34±6.59 years old, the mean SE was -11.93±3.74D, the mean LT was 3.82±0.29mm; With the increase of age, LT became thicken, ASL, LP, RLP were statistically different, except ACD. With the increase of AL, LT became thicken, ACD became deepened, ASL was longer, LP value was bigger, and RLP value was smaller. High myopia LT was positively correlated with age, AL, and negatively correlated with ACD (P=0.000), the better associated with age (partial correlation coefficient r=0.551, P=0.000), and with the age growth, LT increased about 0.025 mm/year.Conclusions For high myopia patients, the lens thickness increased with age, AL, the posterior lens surface and the lens center moved backward. Due to the obvious growth of axial length, the relative lens position moved forward.
