经上颌骨外旋入路的颅中窝底近中线区域肿瘤的手术治疗

摘　　要：目的探讨采用上颌骨外旋入路手术切除颅中窝底近中线区域肿瘤的方法及效果。方法应用经上颌骨外旋入路对8例颅中窝底近中线区域肿瘤患者进行手术治疗。总结分析了该手术径路的方法、适应症、优势、注意事项及治疗效果。结果采用该手术入路均使颅中窝底近中线区域肿瘤获得足够的暴露,所有肿瘤均被安全而完整地切除,术后切口愈合良好,颌面部无明显畸形及功能障碍,其中2例术后行放疗。随访时间1~1.5年,未见肿瘤复发。结论经上颌骨外旋入路切除颅中窝底近中线区域肿瘤具有显露充分、肿瘤切除彻底、手术安全、降低并发症的优点,是一种理想可靠的手术入路。Objective To investigate the method and effect after surgical removal of the tumornear the midline of the middle cranial fossa via the maxillary swing approach. Methods Eight cases with tumor near the midline of the middle cranial fossa were involved in this study. The patients were operated with the maxillaryswingapproach. The access and surgical approaches,indications, advantages, considerations, andeffectof surgery were summarized and analyzed. Results Enough exposure was obtained in this study. All cases of near the midline of the middle cranial fossatumors were safely and completely removed. The patients healed with no severe dentofacial malformation and dysfunction. Two cases received post--operation radiotherapy. Within the 1 ~ 1.5 years follow - up, no recurrence was found. Conclusion Good expo- sure,complete tumor resection and safe with minimally complication rate can be achieved via the maxillary swing ap- proach for tumors near the midline of the middle cranial fossa. It is an ideal and reliable surgical approach.
