认知功能训练配合多奈哌齐治疗对脑卒中后认知障碍患者生活质量的影响

摘　　要：目的探讨认知功能训练配合多奈哌齐对脑卒中后认知障碍患者认知功能和生活质量的影响。方法选取2014年6月~2016年1月我院收治的68例脑卒中后认知障碍患者作为研究对象,随机分为对照组和研究组各34例。两组患者均给予溶栓、抗炎、脱水、降颅压等常规治疗,对照组给予口服多奈哌齐治疗,而研究组则在对照组的基础上加用认知功能训练治疗。用简易精神状态量表(MMSE)、蒙特利尔认知评估量表(MoCA)和日常生活能力量表(ADL)来评估两组患者的临床疗效。采用脑卒中专门生活质量量表(SS-QOL)对两组患者的生活质量进行评估。结果研究组患者治疗后的MMSE和MoCA评分显著高于对照组(P<0.05);研究组患者治疗后的ADL评分显著低于对照组(P<0.05);研究组患者治疗后6个月的SS-QOL评分显著高于对照组的(P<0.05)。结论认知功能训练配合多奈哌齐可以有效改善脑卒中后认知障碍患者的认知功能,提高患者的日常生活能力和生活质量。Objective To discuss the effect donepezil combined with cognitive function training to the quality of life and cognition function in patients with cognition impairment after cerebral stroke. Methods 68 patients with cogni- tion impairment after cerebral stroke from June 2014 to January 2016 in our hospital were selected and divided into the control group and the research group randomly, each group had 34 patients. The patients in control group were treated with donepezil only, while the patients in research group were treated with donepezil combined cognitive function training. The scores of MMSE, MoCA and ADL before and after treatments between two groups were compared. Results After treatments, the scores of MMSE and MoCA in research group were higher than those in control group （P 〈 0.05 ）; The scores of ADL in research group were lower than that those in control group （P〈0.05） . The scores of SS-QOL in research group were higher than those in control group （P 〈 0.05 ） . Conclusion The donepezil combed with cognitive function training can improve cognitive function, activity of daily living and quality of life in patients with cognition impairment after cerebral stroke.
