摘  要:目的探讨nm23-H1、VEGF-C和CD31联合检测在甲状腺乳头状癌(PTC)的诊断与淋巴结转移方面的作用及可能的分子机制。方法采用免疫组化方法检测110例PTC手术切除标本中nm23-H1、VEGF-C和CD31表达情况,同时以60例结节性甲状腺肿伴乳头状增生组织和20例正常甲状腺组织作为对照。结果nm23-H1、VEGF-C和CD31在PTC组织中阳性表达率分别为59.1%、68.2%和64.1%,明显高于良性乳头状增生组(35%、33.3%和43.1%),其中nm23-H1在无淋巴结转移组的阳性率(73.5%)明显高于淋巴结转移组(35.7%)(P<0.05);而VEGF-C和CD31在淋巴结转移组的阳性率(90.5%和78.1%)明显高于无淋巴结转移组(54.4%和51%)(P<0.05)。且PTC组织中nm23-H1的表达与VEGF-C的表达呈负相关(r=-0.63,P<0.05)。结论在PTC组织中,nm23-H1可能通过对G蛋白的控制来影响VEGF-C、CD31的表达及淋巴结的转移,三者联合检测为早期判断PTC有/无淋巴结转移及转移潜能提供重要依据。Objective To study the correlation expression of VEGF-C, nm23-H1 and CD31 in papillary thyroid carcinoma ( PTC), to explore the effect of three kinds of antibody on diagnosis and lymph node metastasisof PTC, and to explore the possible molecular mechanisms and metastatic potential. Methods Immunohistochemistry was used to test the expression of the VEGF-C, nm23-H1 and CD31 in 110 cases of PTC. At the same time, 60 cases of nodular goiter with papillary hyperplasia and 20 cases of normal thyroid tissue were used as control. Results The expression of CD31, VEGF-C, nm23-H1 in PTC tissues (59. 09% , 68.18%, 64. 13±7. 01, respectively) was significantly higher than that of benign papillary hyperplasia group (35% , 33.33% , 43.03 + 4. 25, respectively). In contrast with CD31 and VEGF-C, the positive expression rate of nm23-H1 in the without lymph node metastasis group (73.53%)was significantly higher than that in the group with lymph node metastasis (35.71% ) (p 〈 0. 05 ). The expression of nm23-H1 was negatively correlated with VEGF-C and CD31 inPTC (r= - 0.63, P〈0.05). Conclusion Nm23-H1 may through affect the function of G protein interfere with the expression of VEGF-C and CD31 and lymph node metastasis in the PTC. The combined detection of three provides an important reference for the early determining whether there are lymph node metastasis and potential of metastasis of PTC.
