雷替曲塞联合顺铂腹腔内热灌注治疗恶性腹水的临床研究

摘　　要：目的观察雷替曲塞联合顺铂腹腔内热灌注治疗恶性腹水的近期疗效和安全性。方法 40例经腹水液基细胞学确诊为恶性腹水患者随机分成治疗组(n=20)和对照组(n=20)。治疗组行雷替曲塞4 mg和顺铂60 mg·m^(-2)腹腔内热灌注,每3周1次,共2~4次,对照组仅给予顺铂60 mg·m^(-2)腹腔内热灌注治疗,对比2组治疗效果及不良反应。结果治疗组有效率(85.0%)高于对照组(65.0%),差异有统计学意义(P<0.05)。2组治疗后KPS评分均较治疗前有明显改善(P<0.05);治疗后治疗组KPS评分优于对照组,但差异无统计学意义(P>0.05)。治疗组无明显雷替曲塞相关不良反应。结论雷替曲塞联合顺铂腹腔内热灌注化疗疗效优于顺铂腹腔内热灌注,且并未增加不良反应,该方案是一种安全有效的控制恶性腹水的方法。Objective To investigate the effects of raltitrexed plus cisplatin through intraperitoneal hyperthermic chemoperfusion in the treatment of malignant ascites. Methods The 40 patients with malignant ascites were ran- domly divided into the treatment group and the control group. In the treatment group, the 20 patients were treated with raltitrexed 4 mg and cisplatin 60 mg·m-2 by intraperitoneal hyperthermic chemoperfusion about two to four cy- cles every three weeks. In the control group,the 20 patients were just treated with cisplatin 60 mg · m-2 by intraper- itoneal hyperthermic chemoperfusion. The therapeutic effects of the two groups was compared. The related complica- tion and adverse reaction were recorded. Results The effective rate in the treatment group was 85.0%, and was significantly higher than that in the control group （65.0% ,P 〈 0.05）. The KPS scores in the treatment group were higher than those in the control group, but there was no statistical difference （P 〉 0. 05 ）. No rahitrexed related com- plication and adverse reaction were observed in the treatment group. Conclusion Raltitrexed plus cisplatin through intraperitoneal hyperthermic chemoperfusion is better than cisplatin through intraperitoneal hyperthermic chemoper- fusion, and the related adveres reaction is not increased. So it is one kind of safe and high-efficient therapy instru- ment to malignant aseites.
