马来酸咪达唑仑片联合富马酸喹硫平治疗老年痴呆并发睡眠障碍临床观察

摘　　要：目的研究马来酸咪达唑仑片(力月西)联合富马酸喹硫平治疗老年痴呆并发睡眠障碍的临床效果。方法选取2012年2月—2015年10月首都医科大学良乡教学医院治疗的124例老年痴呆并发睡眠障碍病人为研究对象,随机分为观察组与对照组,每组62例,对照组单独给予喹硫平,观察组病人在喹硫平治疗的基础上加用咪达唑仑治疗。比较治疗后两组病人匹兹堡睡眠质量指数量表(PSQI)评分,采用BEHAVE-AD量表评价两组病人疗效,采用简易精神状况检查表(MMSE)评价两组病人的精神及认知功能,采用日常生活能力量表(ADL)评价两组病人的日常生活能力。结果治疗后,观察组病人PSQI各项评分均显著低于对照组,差异有统计学意义(P<0.05);与治疗前比较,两组病人治疗后BEHAVE-AD评分均显著降低,观察组显著低于对照组,差异有统计学意义(P<0.05);与治疗前比较,两组病人治疗后MMSE评分均有明显提升,且观察组升高的幅度高于对照组(P<0.05);与治疗前比较,两组病人治疗后的ADL有明显的提升,观察组病人提升更明显(P<0.05)。结论力月西联合富马酸喹硫平治疗老年痴呆并发睡眠障碍的临床效果显著,改善病人睡眠质量更优异,且可以显著提升病人的精神及认知状态,并提高日常生活能力。Obicctivo To study the effect of midazolam （midazolam maleate tablets） with quetiapine fumarate in the treatment of Alzheimer’s disease （AD） with sleep disturbance.Miethcds One hundred and twenty four patients with AD and sleep disturbance form Feb 2012 to Oct 2015 in our hospital were randomly divided into two groups：control group（ n = 62） treated with quetiapine fu- marate,and treatment group（ n = 62） treated with quetiapine fumarate and midazolam.The scores of Pittsburgh Scale Sleep Quality Index （PSQI）’Behavioral Pathology in Alzheimer’s Disease Rating Scale （BEHAVE AD） Mini mental Status Examination table （MMSE）,and the ability of daily living （ADL） score using daily living scale were compared between two groups.Results After treat-ment,the PSQI score in treatment group was significantly lower than that in control group（ P 〈0.05） .Compared with before treat-ment,the BEHAVE AD scores were significantly decreased in two groups,while the scores of behavioral disorders,fear, rhythm dis-order in treatment group was lower than that in control group （ P 〈0.05） .Compared with before treatment, the MMSE scores in two groups were significantly increased,which was higher in treatment group than that in control group （ P 〈0.05） .ADL scores in two groups were significantly increased .which was higher in treatment group than that in control group （ P〈 0.05） .Conclusion Midazo-lam and quetiapine fumarate can improve the sleep quality,the mental ,cognitive and life skills in patients with AD and sleep disturb-ance.
