吉非替尼联合立体定向放疗对局部晚期非小细胞肺癌的近期疗效

摘　　要：目的探讨吉非替尼联合立体定向放疗对局部晚期非小细胞肺癌(NSCLC)的近期疗效。方法收集2013年5月~2016年6月间本院收治的局部晚期NSCLC患者200例,回顾治疗方案,将入组患者分为对照组103例,接受单纯立体定向放疗;观察组97例,接受吉非替尼联合立体定向放疗。治疗3月后,评价两组患者的近期疗效,采用酶联免疫吸附法(ELISA)检测血清肿瘤标志物、血管新生指标含量。结果治疗3月后,观察组治疗有效率显著高于对照组(P<0.05);观察组血清肿瘤标志物癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、人鳞状细胞癌相关抗原(SCCAg)、细胞角蛋白19片段(CYFRA21-1)含量,血管新生指标碱性成纤维细胞生长因子(b FGF)、基质金属蛋白酶-9(MMP-9)、血管内皮生长因子(VEGF)含量均低于对照组,差异有统计学意义(P<0.05)。结论吉非替尼联合立体定向放疗可提高局部晚期NSCLC患者的近期疗效,降低肿瘤恶性程度。Objective To study the short-term efficacy of gefitinib combined with stereotactic radiotherapy in the treatment of local advanced non-small cell lung cancer （NSCLC） patients. Methods 200 cases of local advanced non-small cell lung cancer patients in our hospital during May 2013 and June 2016 were chosen as research subjects. The treatment plans were reviewed and all patients were divided into control group （n= 103） and observation group （n=97）. The control group underwent stereotactic radiotherapy only, while the observa- tion group underwent gefitinib combined with stereotactic radiotherapy. The short-term efficacy of two groups was evaluated at three months after treatment. The serum tumor markers and angiogenesis indexes were measured by enzyme linked immunosorbent assay （ELISA）. Re- suits Three months after treatment, the efficiency in observation group was higher than in control group （P〈0.05）. The levels of serum tumor markers such as earcino-embryonic antigen （CEA）, neuron-specific enolase （NSE）, squamous-cell carcinoma-related antigen （SCCAg）, Cytokerantin-19-fragment （CYFRA21-1）, and the levels of angiogenesis indexes such as basic fibroblast growth factor （bFGF）, matrix met- alloproteninases-9（MMP-9）, vascular endothelial growth factor （VEGF） in observation group all were lower than in control group （P 〈 0.05）.Conclusion Gefifinib combined with stereotactic radiotherapy can improve the short-term curative effect of locally advanced NSCLC pa- tients and alleviate the degree of malignancy.
