摘 要:目的揭示肺癌患者放疗前及放疗过程中白细胞介素-8(IL-8)、转化生长因子-β1(TGF-β1)的数值变化与放射性肺损伤(RILI)的关系。方法 103例肺癌患者按常规给予放疗,分别在放疗前、放疗20 Gy、放疗40 Gy及放疗结束时采集血清冻存,采用酶联免疫吸附法(ELISA)检测血清中IL-8、TGF-β1含量,对Ⅱ级及以上RILI进行评价。结果 27例患者发生Ⅱ级及以上RILI,其中Ⅱ级19例,Ⅲ级8例,放疗前血清IL-8含量以及放疗20 Gy时血清TGF-β1含量与RILI的发生有关。结论放疗前IL-8水平较低者及放疗20 Gy时TGF-β1水平升高者,RILI发病的风险增加,即IL-8、TGF-β1的水平与RILI的发生具有相关性,需制定个体化放疗方案。Objective To investigate the relationship of the levels of serum Interleukin-8 (IL-8) and growth transforming factor-be- tal (TGF-β1) with the radiation-induced lung injury (RILI) in patients with lung cancer before and during thoracic radiotherapy. Methods In this study, 103 patients were given routine radiotherapy. Blood samples were collected and frozen before radiotherapy (Pre-RT), at 20 Gy and 40 Gy after radiotherapy (Post-RT), and at the end of radiotherapy respectively. The TGF-β1 and IL-8 levels were measured with enzyme-linked immunosorbent assay (ELISA). RILI was graded according to the Radiation Therapy Oncology Group (RTOG) RILI crite- ria. Grade 2 or more RILI was taken as the main end point. Results Among the 103 patients, 27 of them developed Grade 2 or more RILI, including 19 patients in Grade 2 and 8 patients in Grade 3. The development of RILI was correlated with the TGF-β1 level at 20 Gy Post- RT and IL-8 level at Pre-RT. Conclusion The risk of RILI was slightly increased in patients who had the rising level of TGF-β1 at 20 Gy Post-RT and the lower level of IL-8 at Pre-RT. That is, the levels of TGF-β1 and IL-8 had correlation with incidence of RILI.
