摘 要:目的:探讨应用Pentacam系统引导个性化植入非球面人工晶状体,使全眼球差接近+0.1 mm方法的可行性。方法:选择行白内障超声乳化联合人工晶状体植入的患者72例90眼随机分为观察组(37例45眼)和对照组(35例45眼),术前均应用Pentacam三维眼前节分析系统测量患者6 mm瞳孔直径下的角膜球差。观察组以术后全眼球差接近+0.1 mm为原则,分别植入Tecnis ZCB00、Acry Sof IQ、ADAPT-AO非球面人工晶状体。术后3个月,检查两组患者裸眼视力、最佳矫正视力、角膜球差和全眼球差。结果 :两组患者的裸眼视力均显著提高(P<0.05),但裸眼视力、最佳矫正视力对比差异无统计学意义(P>0.05);两组全眼眼差实际测量值差异有统计学意义(P<0.05);不同类型非球面人工晶状体术前预测与术后实际测量全眼眼差比较差异均无统计学意义(P>0.05)。结论 :应用Pentacam系统术前测量角膜球差,引导个性化植入非球面人工晶状体,有效降低全眼球差,具有一定的可行性。Objective: To explore the feasibility of selectively targeting +0.1 μm total spherical aberration by personalized aspheric intraocular lens implantation. Methods: Seventy-two cases (90 eyes) with personalized aspheric intraocular lens implantation were selected and randomly divided into an observation group (37 cases, 45 eyes) and a control group (35 cases, 45 eyes). Before operation, the corneal spherical aberration with 6 mm pupil diameter was measured by Pentacam system. According to the principle as the total spherical aberration close to +0.1 μm, three different aspheric intraocular lens such as Tecnis ZCB00, AcrySof IQ and ADAPT-AO were implanted in the observation group. At postoperative 3 months, the uncorrected visual acuity, best corrected visual acuity, corneal spherical aberration and total ocular aberration were compared between the two groups. Results: The uncorrected visual acuity in the two groups was improved with significant differences (P〈0.05). There was no statistically significant differences in the uncorrected visual acuity and best corrected visual acuity between the two groups (P〉0.05). The differences of total ocular aberrations were statistically significant between the two groups (P〈0.05). There were no significant differences in total ocular aberration and preoperative corneal spherical aberration in the observation group (P〉0.05). Conclusion: The personalized aspheric intraocular lens implantation guided by Pentacam system is feasible.
