微创侧脑室引流联合尿激酶灌注治疗自发性脑室出血临床分析

摘　　要：目的探讨微创侧脑室引流联合尿激酶灌注治疗自发性脑室出血的临床疗效。方法回顾性分析我院神经外科于2010年1月至2015年12月期间收治的72例自发性脑室出血患者的临床资料,将其中12例采用内科保守治疗的患者纳入对照组,60例采用微创侧脑室引流联合尿激酶灌注治疗的患者纳入观察组,对患者进行6个月的随访,比较两组患者的脑积水发生率及临床疗效。结果两组患者随访6个月时,观察组患者的治疗良好率为66.67%,明显高于对照组的33.33%,差异有统计学意义(P<0.05);观察组患者的脑积水发生率为16.67%,明显低于对照组的41.67%,差异有统计学意义(P<0.05)。结论微创侧脑室穿刺引流联合尿激酶灌注治疗自发性脑室出血临床疗效确切,且可以降低脑积水并发症的发生率,进而改善患者的预后。Objective To investigate the clinical effect of minimally invasive lateral external drainage combined with urokinase infusion in the treatment of spontaneous ventricular hemorrhage. Methods The clinical data of 72 cases of spontaneous intraventricular hemorrhage patients treated in Department of Neurosurgery of our hospital from January 2010 to December 2015 was retrospectively analyzed. Twelve cases with conservative medical treatment were included into the control group, and sixty cases with lateral external drainage combined with urokinase infusion treatment were included into the observation group, which were followed up for 6 months. The incidence of hydrocephalus and clinical curative effect were compared between the two groups. Results After 6 months of follow-up, the good rate of treatment in the observation group was 66.67%, which was significantly higher than 33.33% in the control group（P〈0.05）. The incidence of hydrocephalus in the observation group was 16.67%, which was significantly lower than 41.67%in the control group（P〈0.05）. Conclusion Minimally invasive lateral external drainage combined with urokinase infusion in the treatment of spontaneous ventricular hemorrhage has a definite curative effect, which can reduce the incidence of hydrocephalus complications, thereby improving the prognosis of patients.
