小瞳孔白内障手术中内置式虹膜扩张器的应用

摘　　要：目的 评价OASIS型内置式虹膜扩张器在小瞳孔白内障手术中临床应用的效果.方法 小瞳孔白内障晶状体超声乳化联合人工晶体植入术64例（64眼）,包括两个组：OASIS组,32例（32眼）,术中以OASIS型内置式虹膜扩张器扩大瞳孔;传统手术组,32例（32眼）,术中未借助扩瞳器械进行扩瞳.两组的手术时间、有效超乳时间、术后前房反应及最佳矫正视力进行比较,并问卷调查手术难易度及患者术后满意度.结果 Ⅲ、Ⅳ级核者手术时间OASIS组少于传统手术组,差异均有统计学意义（P=0.004,0.001）;Ⅱ、Ⅲ、Ⅳ级核者有效超乳时间OASIS组均少于传统手术组,差异有统计学意义（P=0.003,0.000,0.000）;OASIS组较传统组术后角膜水肿少、角膜内皮损伤少、房水闪光轻、视力恢复快,差异均有统计学意义（P〈0.05）;问卷调查OASIS组较传统组手术难度低、患者术后满意度高,差异均有统计学意义（P〈0.05）.结论 OASIS型内置式虹膜扩张器在小瞳孔白内障的超声乳化术中应用安全有效.Objective To evaluate the clinical application efficacy of OASIS type built-in iris dilator in ultrasonic phacoemulsification for small pupil cataract.Methods Sixty-four eyes of 64 cases of small pupil catanact underwent phacoemulsification and introcular lens implantation were divided into 2 groups： OASIS group ,32 cases,OASIS type built-in iris dilator applied in surgery;conventional surgery group,32 cases,without the application of built-in iris dilator.The operation time ,efficient ultrasonic emulsification time ,anterior chamber reaction and best corrected visual acuity after operation were compared between the two groups.The operation difficulty level and patients satisfaction were investigated by questionnaire between the two group.Results The operation time of grade Ⅲ, Ⅳ nuclear in OASIS group were less than conventional surgery group.The differences were statistically significant （P=0.004,0.001）.The effective phaco time of grade Ⅱ, Ⅲ, Ⅳ nuclear in OASIS group were less than convetional surgery group.The differences were statistically significant（P=0.003,0.000,0.000）.The postoperative corneal endothelial density in OASIS group was higher than that in conventional group （P〈0.05）.The OASIS group revealed less postoperative corneal edema and better visual acuity （P〈0.05）.Questionnaire showed operation difficulty in the OASIS group was reduced, postoperative satisfaction was higher,compare with convetional surgery group （P〈0.05）.Conclusion The built-in iris dilator is safe and effective in the application of phacoemulsification in small pupil cataract.
