皮肤外伤诱发肿胀性红斑狼疮1例

摘　　要：患者女,56岁。左侧面部红斑、肿胀伴疼痛1.5年。发病前曾被仙人球刺伤。抗核抗体颗粒型1∶100阳性。皮损直接免疫荧光检查阴性,皮损组织病理示:表皮基底细胞液化变性,血管及附属器周围淋巴细胞浸润,阿新蓝染色真皮胶原纤维间可见大量黏蛋白沉积。诊断:肿胀性红斑狼疮。予甲泼尼龙(逐渐减量至停药,共计1.5年)、羟氯喹(1年)、雷公藤多苷片(1个月)口服,并予他克莫司软膏外搽1年,皮损已消退,未见遗留瘢痕。A 56-years-old woman presented with a painful swelling erythema on the left face for one and a half years.She was injured by the prickly pear prior to lesion eruption.Positive anti-nuclear antibody titer was 1：100 for particle type.Cutaneous direct immunofluorescence was negative.The histopathological examination of the skin lesion showed liquefaction degeneration of basal cells, perivascular and periappendicular infiltrate of lymphocytes.Alcian blue stain showed abundant mucin deposition in collagen bundles of the dermis.The diagnosis was lupus erythematosus tumidus.She was treated with oral methylprednisolone for 1.5 years（gradual reduction of dose to zero）, hydroxychloroquine sulfate（1 year）, and glucosidorum tripterygll totorum（1 month）, and topical tacrolimus ointment for 1 year.The lesion completely resolved without scarring.
