青年乳腺癌患者术后化疗期抑郁的影响因素分析

摘　　要：目的了解青年乳腺癌患者术后化疗期抑郁的发生状况,探讨治疗过程中抑郁的影响因素。方法采用自行设计的一般资料问卷、M.D.Anderson症状调查量表(MDASI)和抑郁自评量表(SDS)对150例患者在术后化疗期间进行问卷调查。结果有66%的患者在术后化疗期存在不同程度的抑郁障碍,其抑郁水平高于国内常模(P<0.001)。多因素分析结果显示,家庭关系、对病情的主观感受、疾病造成的经济负担是青年乳腺癌患者术后化疗期抑郁的主要影响因素(P<0.001)。在化疗导致的不良反应中,恶心、疲劳、呕吐、睡眠不安是导致患者抑郁的主要影响因素(P<0.001)。结论术后化疗期是青年乳腺癌患者发生抑郁的易感时期,医护人员可根据主要影响因素制定相应的预防护理措施,以降低患者的抑郁水平。Objective To investigate incidence of depression in young patients with breast cancer during postoperative chemotherapy period,so as to analyze influencing factors of depression during the course of treatment.Methods 150 patients who underwent chemotherapy after operation were investigated by self-designed general information questionnaire,M.D.Anderson Symptom Inventory（MDASI） and Self-rating Depression Scale（SDS）.Results 66% of patients have depressive disorder to various levels,and the level of depression was higher than the domestic norm（P〈0.001）.Multiple regression analysis showed that family relationship,subjective feeling to the disease,economic burden caused by disease were the main influencing factors of depression（P〈0.001）.In chemotherapy side effects,nausea,fatigue,vomiting,sleep disturbances were the main influencing factors of depression（P〈0.001）.Conclusion Postoperative chemotherapy is a susceptible period of depression in young patients with breast cancer,medical staff should pay more attention to the main influencing factors and develop appropriate preventive measures to reduce the level of depression.
