超声乳化吸出联合人工晶状体植入术治疗白内障并发症高危因素分析

摘　　要：目的探讨超声乳化吸出联合人工晶状体植入术治疗白内障并发症的高危因素。方法选择2012年1月~2015年12月在我院行白内障超声乳化吸出术联合人工晶体植入患者838例,对其临床资料进行回顾性分析。记录患者术中术后并发症情况,根据患者术后是否发生并发症分为并发症组与无并发症组。记录患者一般资料、临床资料、系统性疾病史、外伤史、既往眼部疾病史、既往眼部手术史等,分析患者并发症高危因素。结果共146例发生术中术后并发症。单因素分析结果显示,并发症组高血压、糖尿病、眼部手术史、小睑裂、结膜松驰、角膜老年环、玻璃体混浊、Emery分级Ⅳ~Ⅴ级比例显著高于无并发症组,差异有统计学意义(P<0.05)。多因素分析结果显示,高血压、糖尿病、眼部手术史、小睑裂、结膜松驰、角膜老年环、玻璃体混浊、Emery分级Ⅳ~Ⅴ级是白内障超声乳化吸出联合人工晶体植入术并发症的独立危险因素(P<0.05)。结论白内障超声乳化吸出联合人工晶体植入术术中及术后并发症较多,而高血压、糖尿病等全身疾病、眼部疾病史以及手术史均是并发症的高危因素,在临床操作过程中,对于存在高危因素的患者,应充分做好手术准备,做出合适的应对措施,以减少并发症的发生。Objective To investigate the high risk factors of cataract complications treated by ultrasonic phacoemulsification combined with intraocular lens implantation. Methods The clinical data of 838 patients with cataract pha- coemulsification combined with intraocular lens implantation in our hospital from January 2012 to December 2015 were retrospectively analyzed. The intraoperative and postoperative complications of the patients were recorded. According to whether occurrence of complications, the patients were divided into complications group and non-complication group. The general information of patients, clinical data, systemic disease history, trauma history, past eye disease history, past eye surgery history were recorded. The high risk factors of complications in patients were analyzed. Results A total of 146 patients had postoperative complications. Univariate analysis showed that the proportions of hypertension, diabetes mellitus, eye surgery, small palpebral fissure, conjunctival relaxation, corneal arcus senilis, vitreous opacity and Emery grade Ⅳ- Ⅴ in the complications group were significantly higher than those of non-complication group. And the difference was statistically significant （P〈0.05）. Multivariate analysis showed that hypertension, diabetes, eye surgery history, small patpebral fissure, conjunctival relaxation, corneal arcus senilis, vitreous opacity, and emery grade grade Ⅳ- Ⅴ were the independent risk factors of complications of cataract phacoemulsification combined with intraocular lens im- plantation（P〈0.05）. Conchtsion Phacoemulsification combined with intraocular lens implantation has many complications during and after operation. The generalized diseases such as hypertension and diabetes mellitus, the history of ocular disease and the history of surgery are the risk factors of complications.
