不同剂量紫杉醇联合卡铂治疗局部晚期宫颈癌同期放化疗的疗效观察

摘　　要：目的:探讨不同剂量紫杉醇联合卡铂治疗局部晚期宫颈癌同期放化疗效果。方法:选取2013年7月-2015年1月本院收治局部晚期宫颈癌患者75例,按随机数字表法分为A组和B组,两组患者采用完全相同的放射治疗方案:体外照射采用全盆腔仰卧位直线加速器盆腔大野DT 40 Gy。Ⅰb2期盆壁野将中央挡铅改为盆腔四野加量照射至DT 50 Gy,Ⅱa期及以上加量至55 Gy,开始盆腔四野照射后每周局部后装铱192照射1次,每次剂量7 Gy,共6次,A点剂量达82 Gy,B点剂量50~55 Gy。化疗方案:A组患者在放射治疗的第1天同步静脉滴注紫杉醇135 mg/m^2+注射用卡铂(AUC=5-6),3周为一疗程,共2个疗程;B组在每周放射治疗的第1天同步静脉滴注紫杉醇35 mg/m^2+注射用卡铂(AUC=2-3),1周为一疗程,共6个疗程。观察比较两组的临床疗效及不良反应。结果:两组治疗总有效率比较,差异无统计学意义(P>0.05)。两组不良反应发生情况比较,差异均无统计学意义(P>0.05)。两组1、2年生存率比较,差异均无统计学意义(P>0.05)。结论:小剂量紫杉醇联合卡铂同期放化疗治疗局部晚期宫颈癌的效果与大剂量相当。Objective： To compare the effect of different doses of Paclitaxel combined with Carboplatin concurrent chemoradiotherapy for locally advanced cervix cancer.Method： A total of 75 cases who were diagnosed as locally advanced cervical cancer from July 2013 to January 2015 were divided into A group and B group.Twogroups were treated with the same radiotherapy project： the same pelvic irradiation DT 40 Gy with linear accelerator and then boost to DT 50-55 Gy with a block to protect the rectum based on the different clinical stage.After that, a after loading irradiation by iridium 192 were given as a dose of 7 Gy, a total of 6 times, aiming the dose of A point 82 Gy and B point 50-55 Gy.The chemotherapy regimen： A group was treated with Paclitaxel 135 mg/m2＋ Carboplatin （ AUC=5-6 ） for injection every 21 days for two courses; B group was treated with Paclitaxel 35 mg/m2＋ Carboplatin （ AUC=2-3 ）for injection every week for 6 courses.Result： there was no statistical significance in total efficiency of two groups （ P〉0.05 ） .There was no significant difference in adverse reactions of two groups （ P〉0.05 ）. There were no significant differences in survival rate of 1 year and 2 years between the two groups （ P〉0.05 ） . Conclusion： Small doses of Paclitaxel and Carboplatin chemotherapy is similar in curative effect to large doses.
