胆囊癌患者癌组织中共刺激因子B7-H1和B7-H3的表达水平及意义

摘　　要：目的探讨共刺激因子B7-H1和B7-H3在胆囊癌患者癌组织中的表达及临床意义。方法选取2006年8月至2010年8月间陕西省第二人民医院收治的40例胆囊癌患者,取手术切除的癌组织标本为观察组,取癌旁正常组织20例作为对照组,采用免疫组织化学SP法检测两组标本中B7-H1和B7-H3蛋白的表达水平。结果观察组标本中B7-H1和B7-H3蛋白的表达水平显著高于对照组标本。在观察组标本中表达定位主要浸润胞浆位置,染色呈淡黄色、棕黄色和棕褐色,观察组标本中B7-H1表达阳性细胞以高度浸润为主。胆囊癌(Ⅰ+Ⅱ)期中B7-H1蛋白表达阳性率高于胆囊癌(Ⅲ+Ⅳ)期,差异有统计学意义(χ~2=5.714,P=0.02),胆囊癌(Ⅰ+Ⅱ)期中B7-H3蛋白表达阳性率高于胆囊癌(Ⅲ+Ⅳ)期,差异有统计学意义(χ~2=4.224,P=0.04)。胆囊癌患者B7-H1和B7-H3的阳性表达组,其术后总生存率低于阴性表达组。结论 B7-H1和B7-H3的表达水平有助于胆囊癌的早期诊断,同时其含量可能是胆囊癌预后的独立预测因子之一。Objective To explore the expression and clinical significance of B7-H1 and B7-H3 in patients with gallbladder cancer. Methods Forty patients with gallbladder cancer treated at Shaanxi Second Provincial People＇s Hospital from August 2006 to August 2010 were selected. Patients with samples taken from tumors by operation were selected as observation group and 20 patients with samples taken from peritumoral normal tissues were selected as control group. Immunohistochemistry of SP was used to detect the expression level of B7-H1 and B7-H3. Results Expression level of B7-H1 and B7-H3 was higher in the observation group than in the control group. Cytoplasm infiltration was the main existing form of expression in gallbladder carcinoma tissues for the observation group and the dyeing showed faint yellow,pale brown and chocolate-brown. Expression of B7-H1 positive cell was mainly highly invasive in the observation group. Expression of B7-H1 and B7-H3 was higher in observation group than in the control group. B7-H1 positive rate was higher in stage Ⅰ and stage Ⅱ gallbladder cancer than in stage Ⅲ and Ⅳ gallbladder cancer（ χ^2= 5. 714,P = 0. 02）. B7-H3 positive rate was higher in stageⅠ and stage Ⅱ gallbladder cancer than in stage Ⅲ and stage Ⅳ gallbladder cancer（ χ^2= 4. 224,P = 0. 04）. Postoperation overall survival rate was obviously lower in patients with positive expression of B7-H1 and B7-H3 than with negative expression. Conclusion Expression level of B7-H1 and B7-H3 may help to improve early diagnosis of gallbladder cancer and possibly was one of the independent predictors of gallbladder prognosis.
