序贯放化疗治疗CA19-9升高的老年晚期胆管癌患者的临床疗效

摘　　要：目的探讨序贯放化疗治疗CA19-9升高的老年晚期胆管癌患者的临床疗效。方法选取2012年10月至2015年10月间黄岛区人民医院收治的行序贯三维适形放疗治疗的24例晚期胆管癌患者。治疗完成后,收集患者相关临床指标,统计分析患者临床疗效和不良反应情况。结果治疗结束后,患者完全缓解3例(12.5%),部分缓解16例(66.7%),疾病稳定3例(12.5%),疾病进展2例(8.3%),总有效率为79.2%。有21例患者疼痛减轻或消失,生活质量提高。20例患者CA19-9水平降低,患者中位生存期为16.1个月。结论吉西他滨联合雷替曲塞序贯三维适形放疗治疗CA19-9升高的晚期胆管癌患者,具有较好的近期疗效,且不良反应可耐受。Objective To explore the clinical characteristics of elderly CA19-9 elevated patients with advanced bile duct cancer receiving sequential chemoradiotherapy. Methods From October 2012 to October 2015,24 patients with advanced bile duct cancer who were treated with sequential three-dimensional conformal radiotherapy at People＇s Hospital of Huangdao District were selected. After the treatment,the clinical data of patients were collected,and the clinical efficacy and adverse drug reaction were analyzed.Results After treatment,complete relief（ CR） was found in 3 patients（ 12. 5%）,partial relief（ PR） was found in 16 patients（ 66. 7%）,stable disease（ SD） was found in 3 patients（ 12. 5%）,progressive disease（ PD） was found in 2 patients（ 8. 3%） and the overall effective rate is 79. 2%. Pain relief or pain elimination was found in 21 patients with quality of life improved. Serum CA19-9 levels decreased in 20 patients. The median survival time was 16. 1 months for all the patients. Conclusion Gemcitabine combined with raltitrexed and three-dimensional conformal radiotherapy in the treatment of advanced carcinoma of the bile duct with elevated CA19-9 has a good short-term efficacy and tolerable adverse effects.
