摘 要:目的探讨相关肿瘤标志物组合在卵巢癌诊断中的应用价值。方法收集本院42例卵巢癌患者、75例良性卵巢疾病患者和50例健康体检者的静脉血,应用多肿瘤标志物蛋白芯片检测技术测定CA125、CEA、CA19-9、CA24-2等12种肿瘤标志物的含量,通过Logistic回归建立模型,绘制ROC曲线并计算曲线下面积来评价各肿瘤标志物的诊断价值。结果卵巢癌组的CA125、CEA、CA19-9、CA24-2、FER的检测结果及阳性率明显高于良性卵巢疾病组和健康体检组,差异有统计学差异(P<0.05)。其余肿瘤标志物在3组及2组间比较差异无统计学意义(P>0.05)。CA125、CEA、CA19-9、CA24-2的曲线下面积分别为0.796、0.693、0.726、0.652。4种肿瘤标志物联合检测曲线下面积为0.920,敏感性和特异性分别为88.81%和78.20%。结论单独检测CA125对卵巢癌的诊断有一定价值。CA125、CEA、CA19-9、CA24-2联合检测可为卵巢癌的临床诊断提供有效参考价值。Objective To explore the application value of tumor markers in the diagnosis of ovarian cancer. Methods 12 kinds of common tumor markers were detected by multi tumor markers protein chip technology in 42 patients with ovarian cancer, 75 patients with benign ovarian disease and 50 cases of healthy physical as controls. We established regression equa- tions, drew ROC curve and compared the area under curve (AUC) to evaluate the diagnostic value of each indicator of tumors. Results The detection results and positive rates of carbohydrate antigen 125 ( CA125 ) ; carbohydrate antigen 19 - 9 ( CA19 - 9) ; carbohydrate antigen 24 -2( CA24 -2) ; carcinoembryonic antigen( CEA), and FER in ovarian cancer group were higher than those in benign ovarian tumor group, and the differences were statistically significant( P 〈0.05 ). There was no statistical significance on the difference of other tumor markers among the three or two groups (P 〉 0.05 ). The AUC of CA125, CEA, CA19 - 9 and CA24 - 2 were 0.796, 0. 693 , 0. 726, 0.652, respectively. The AUC of the combination of the four tumor mark- ers was O. 920. The sensitivity and specificity of combined detection were 88.81% and 78.20%, respectively. Conclu- sion Detection of CA125 alone has a certain value in the diagnosis of ovarian cancer. The combined detection of CA125, CEA, CA199 and CA242 can provide reference value for clinical diagnosis of ovarian cancer.
