摘 要:目的 探讨经腹剪切波成像(shear-wave,SWV)技术对前列腺良恶性病变诊断的临床价值.方法 应用SWV技术对60例前列腺占位性病变患者进行检查,测值与术后病理结果对照,鉴别其良恶性.结果 SWV技术检测的前列腺恶性占位病变与前列腺良性占位病变组比较,剪切波速差异有统计学意义(P<0.01);前列腺良性占位与前列腺正常组的剪切波速比较差异具有统计学意义(P<0.017).以剪切波速大于2.87 m/s作为前列腺恶性占位的诊断标准,SWV鉴别前列腺良恶性占位的敏感性为80.0%,特异性为66.7%.结论 SWV技术可以为前列腺良、恶性病变的筛查提供更多的信息和依据.Objectives To investigate the the value of shear wave (SWV) for early diagnosis of prostate benign and malignant lesion.Methods 60 patients with prostate lesions were examined by SWV,technology and postoperative pathology was measured and compared for differentiating the benign and malignant tumors.Results SWV technology for detection of prostatc malignant lesions and benign lesions were compared between groups,there was significant difference in shear wave velocity (P 〈 0.001).There was significant difference in shear wave velocity between the prostate benign mass and normal prostate groups (P 〈0.017).With the SWV 〉 2.87 m/s as diagnostic point for benign and malignant prostate lesions,the positive predictive value and specificity for malignancy was 86.7% and 66.7%,respective.Conclusions SWV technology will provide more information and basis for early differential diagnosis of prostate benign and malignant lesions.
