摘 要:目的目前对于阿尔茨海默病(Alzheimer's disease,AD)患者脑内血氧信号的非线性特性研究较少。文中旨在探讨通过多尺度熵(multiscale entropy,MSE)方法表述AD患者脑内动态血氧信号的复杂度特征,并评估其对于阐释AD病理生理机制的作用。方法收集2010年12月至2015年9月于首都医科大学宣武医院神经内科记忆门诊就诊的AD患者24例,为AD组;招募来自社区的性别、年龄及受教育程度与AD组相匹配的健康老年人31名为健康对照组。采集31名健康对照者以及24例AD患者的静息态功能性近红外成像数据,分析比较各组血氧信号的MSE特征,并分析其与认知功能的关系。结果 AD组的MSE全脑均值小于健康对照组[(6.23±0.32)vs(7.13±0.41)]。在各个功能网络内,AD患者MSE值较健康对照组在默认网络(6.069 vs 7.222),额顶网络(6.038 vs 6.972),腹侧注意网络(6.238 vs 7.325)、背侧注意网络(6.143 vs 7.035)及视觉网络(5.981 vs 6.919)中均显著降低(P<0.05)。MSE值与整体认知功能在腹侧注意网络中存在边缘显著的正相关(r=0.410,P=0.065)。结论基于MSE的分析方法有潜力成为表述及阐释AD患者异常皮层血氧信号复杂度的新手段。Objective The nonlinear properties of hemo-globin signals in Alzheimer's disease (AD) patients are rarely ex-plored. The purpose of this study was to evaluate the multiscale entro-py ( MSE) approach as an aid to characterizing the complexity of he-moglobin signals and elucidating the pathophysiologic mechanisms of AD. Methods We included in this study 24 AD patients and 31 elderly healthy controls that were matched with the patients in gender, age and education. We obtained the resting-state functional near-infra-red spectroscopy ( fNIRS) imaging data from the subjects, analyzed the characteristics of the blood oxygen level-dependent signals by MSE, and assessed their relation with cognition. Results The mean value of MSE was significantly lower in the AD patients than in the normal controls ([ 6.23±0.32] % 仍 [7 .1 3 ± 0 .4 1 ] %, P 〈 0 .0 5 ) ,and so were measures in the default network ( 6.069% 仍 7 .2 2 2 %, P〈0.05) , frontoparietal network (6.038% vs 6.912% ^ P 〈 0 .0 5 ) , ventral attention network (6.238% vs 7.325%, P 〈 0 .0 5 ) , dorsal at-tention network (6.143% vs 7.035%, P〈 0 .0 5 ) , and visual network (5.981% vs 6.919%, P 〈 0 .0 5 ) . There was a marginal significant positive correlation between the MSE values and cognitive performances in the ventral attention networks ( r = 0.41,P = 0.065 ). Conclusion MSE-based analysis is potentially a novel approach to characterizing and understanding the abnormal cortical signal com-plexity in AD patients.
