糖调节受损相关神经病变的临床特点及其影响因素分析

摘　　要：目的探讨糖调节受损(IGR)相关神经病变的临床特点及其影响因素分析。方法选取天津市第三中心医院2015年3月—2016年3月收治的IGR患者60例为IGR组,另选取同期30例健康志愿者为对照组,检测比较两组受试者的空腹血糖、餐后2 h血糖、糖化血红蛋白、低密度脂蛋白、三酰甘油、总胆固醇及体质指数(BMI)、腰围,并检查四肢交感皮肤反应(SSR)潜伏期及波幅,采用多因素Logistic分析IGR相关神经病变的影响因素。结果两组患者上、下肢SSR潜伏期及上肢SSR波幅比较,差异无统计学意义(P>0.05);IGR组患者下肢SSR波幅低于对照组(P<0.01)。两组患者腰围比较,差异无统计学意义(P>0.05);IGR组患者空腹血糖、餐后2h血糖、糖化血红蛋白、低密度脂蛋白、三酰甘油、总胆固醇、BMI高于对照组(P<0.05)。多因素Logistic回归分析结果显示,低密度脂蛋白、三酰甘油、BMI是IGR相关神经病变的危险因素(P<0.05)。结论 IGR患者存在周围神经病变,且以小纤维受损为主,而代谢综合征是IGR相关神经病变发生的危险因素。Objective To investigate the clinical features of impaired glucose regulation （IGR） related peripheral neuropathy and the influencing factors analysis. Methods A total of 60 cases of IGR patients were selected from March 2015 to March 2016 in Tianjin Third Central Hospital as IGR group, 30 cases of healthy volunteers were admitted as control group, the FPG, 2 h PG, HbA1c, LDL, TG, TC and BMI, waist were detected and compared between the two groups, and the incubation period and amplitude of limbs SSR were examined, the influencing factors of IGR related peripheral neuropathy was analysized by muhiariable logistic regression. Results No statistically significant differences of SSR incubation period of upper and lower limbs or SSR amplitude of upper limbs was found between the two groups （P 〉 0.05 ） ; the SSR amplitude of lower limbs of IGR group were lower than control group （ P 〈 0. 05 ）. No statistically significant difference of waist was found between the two groups （P〉0. 05） ; the IGR group of FPG, 2 h PG, HbAlo, LDL, TG, TC and BMI were higher than control group （P 〈0. 05 ）. Multiariable logistic regression showed that LDL, TG and BMI were the risk factors of IGR related peripheral neuropathy （ P 〈 0. 05 ）. Conclusion IGR patients presist peripheral nerve damage, mainly involving small fiber necve of the lower limbs, and the metabolic syndrome is the risk factors of IGR related peripheral neuropathy.
