摘  要:目的总结乳腺浸润性导管癌的乳腺MRI动态增强(DCE-MRI)影像学特征,探讨其的MRI动态增强检查影像及TIC曲线类型。方法 47例患者均采取常规MRI平扫及动态增强扫描,所有病例均经术后病理证实,回顾分析病灶MRI形态、信号特点及时间-信号曲线改变。结果本组47例患者53个病灶,在DCE-MRI影像形态上显示为类圆形7个,分叶状26个,不规则形20个;边缘规则者5个,不规则者20个,毛刺者28个;强化不明显者7个,均匀强化者17个,不均匀强化者29个;向心型强化灶36个,中心-边缘型强化灶17个;于TIC曲线上显示Ⅰ型、Ⅱ型及Ⅲ型分别4例、18例和25例。依据DCE-MRI上病变的形态、边缘和强化、曲线特征进行诊断,诊断正确者43例,诊断准确度为91.14%。结论 DCE-MRI在乳腺浸润性导管癌的诊断上,具有较高价值,可为临床诊断乳腺浸润性导管癌提供有力的影像学依据。Objective To summarize the imaging features of MRI dynamic enhancement (DCE-MRI) on breast infiltrating ductal carcinoma, and discuss its MRI dynamic enhancement detecting image and type of TIC curve. Methods 47 patients took the conventional MRI scanning and dynamic enhanced scanning. Then, all cases were confirmed by postoperative pathology. The morphology, signal characteristics and the change of time-signal curve of MRI were retrospectively analyzed lastly. Results The testing group was consisted by 47 patients with 53lesions. The DCE-MRI images morphology showed 7 round shapes, 26 lobulated shapes and 20 irregular shapes; 5 regular edge, 20 irregular edge and 28 burr edge; 7 less obvious enhancement, 17 homogenous enhancement and 29 heterogeneous enhancement; 36 centripetal-enhancing lesions, 17 center-edge enhancing lesions; the TIC curve showed 4 cases of the type I, 8 cases of the type Ⅱ and 25 cases of the type Ⅲ, respectively. According to the shape, edge and enhancement of the lesions on DCE-MRI, 43 cases were diagnosed correctly, and the diagnostic accuracy was 91.14%. Conclusion DCE-MRI is of high value in the diagnosis of breast infiltrating ductal carcinoma, which can provide a powerful imaging basis for clinical diagnosis of breast invasive ductal carcinoma.
