团体心理治疗对乳腺癌放疗患者心理康复及生存质量的影响

摘　　要：目的:探讨团体心理治疗对乳腺癌放疗患者心理康复及生存质量的影响。方法:对已完成乳腺癌放疗6~8个疗程的患者,预期生存期大于5年的乳腺癌患者共60人进行为期12周的团体心理干预,并在干预前和干预结束后4周给予一般情况调查表、汉密尔顿抑郁量表(HAMD)、症状自评量表(SCL-90)、生存质量量表(WHOQOL-BREF)评估。结果:团体心理治疗患者干预后HAMD评分显著降低(t=2.471,P=0.016),SCL-90中躯体化、强迫、人际关系、抑郁、焦虑、敌对、恐怖因子评分显著性下降(t=2.397,2.225,2.117,5.745,5.789,2.471,2.078;P<0.05)。WHOQOL-BREF总分、生理领域、心理领域因子分显著提高(t=-3.332,-2.000,-1.981;P<0.05)。结论:团体心理治疗明显促进了乳腺癌患者的心理康复,改善了患者生存质量。Objective：To explore the effect of group psychotherapy on psychological rehabilitation and quality of life in patients with breast cancer radiotherapy.Methods：A total of 60 patients with breast cancer who had completed 6—8 course of treatment and their expected lifetime was more than five years were carried out 12—weeks group psycho-therapy,general situation questionnaire,the HAMD scale,SCL—90 scale and quality of life scale were used to assess before intervention and at 4 weeks after intervention.Results：After group psychotherapy,patients＇HAMD score de-creased significantly（t=2.871,P〈0.05）,SCL—90 scale scores in somatization,force,interpersonal relationship,de-pression,anxiety,hostility,terror factor significantly decreased（t=2.397,2.225,2.117,5.745,5.789,2.471,2.078;P〈0.05）.The psychological WHOQOL—BREF score,physiological field and field factor were significantly improved （t=-3.332,-2.000,-1.981;P〈0.05）.Conclusion：Group psychotherapy can obviously promote the psychological re-habilitation of patients with breast cancer,and improve patients＇quality of life.
