摘  要:目的:探讨乙肝肝硬化(LC)发展为原发性肝癌(PLC)的血清肿瘤标志物、 肝酶及蛋白质的变化趋势.方法:追踪血清甲胎蛋白(AFP)阳性、 从乙肝LC发展为PLC时的患者38例,检测其血清的消化肿瘤标志物、 肝酶及蛋白质等指标,对2个时段这些指标的检测结果进行比较和分析.结果:2个时段的血清AFP、 糖类抗原125(CA125)、 糖类抗原19-9(CA19-9)、 丙氨酸氨基转移酶(ALT)、 天门冬氨酸氨基转移酶(AST)、 碱性磷酸酶(ALP)、L-γ-谷氨酰转移酶(GGT)和超敏C-反应蛋白(h-CRP)水平均升高,胆碱酯酶(CHE)和白蛋白(ALB)水平均降低.与LC时比较,PLC时AFP、CA125、ALP、GGT和h-CRP升高更明显(P〈0.01和P〈0.05),CHE和ALB降低更明显(P〈0.01和P〈0.05),CA125、CA19-9、ALP和h-CRP升高的阳性率更高(P〈0.05),CHE和ALB降低的阳性率更高(P〈0.01和P〈0.05).结论:在乙肝LC发展为PLC的过程中,血清AFP、CA125、CA19-9、ALP、GGT、CHE、ALB和h-CRP的变化最明显,可作为这类疾病的监测指标.Objective: To explore the alteration tendency of the sera tumor markers, liver enzymes and proteins in the hepatitis 13 virus infectious liver cirrhosis (LC) became to the primary liver cancer (PLC) . Methods: The 38 cases patients of positive sera alphafetoprotein (AFP) and from the hepatitis B virus infectious LC became to PLC were traced and their sera digestive tumor markers, liver enzymes and proteins were determined. The determined results of these indexes in two different periods were compared and analyzed. Results: The level of sera AFP, carbohydrate antigen 125 (CA125) , carbohydrate antigen 19-9 (CA19-9) , alanine transaminase (ALT) , aspartate transaminase (AST) , alkaline phosphatase (ALP) , y-glutamyl transpeptidase (GGT) and high sensitiviW C-reactive protein 01-CRP) were all increased and the level ofcholinesterase (CHE) , albumin (Alb) were all decreased in two different periods. The rises of AFP, CA125, ALP, GGT and h-CRP were more obvious (/}〈0.01 and P〈0.05) and the reductions of CHE and ALB were more obvious (/}〈0.01 and P〈0.05) when PLC in contrast to LC. The positive rates of rise for CA125, CA19-9, ALP and h-CRP were higher (P〈0.05) and the positive rates of reduction for CHE and ALB were higher (P〈0.01 and P〈0.05) when PLC in contrast to LC. Conclusion: The alterations ofsera AFP, CA125, CA19-9, ALP, GGT, CHE, ALB and h-CRP are most obvious in process of LC became to PLC and these indexes may be used to the monitor indicators of these diseases.
