摘 要:目的探讨徒手心肺复苏及机械心肺复苏过程中颈动脉血流超声的特点和监测作用。方法选取2015年11月~2016年11月我院急诊科心跳骤停的患者28例作为研究对象,随机分为徒手复苏组和机械复苏组,各14例。徒手复苏组给予患者徒手胸外按压30 min,机械复苏组应用心肺复苏机按压30 min。通过超声多普勒技术测量胸外按压过程中初始5 min和终末5 min颈动脉收缩期血流速度峰值均值(PSV)及每分钟按压频率。结果徒手复苏组初始5 min颈动脉PSV平均值高于终末5 min,差异有统计学意义(P<0.01),按压频率达标率高于终末5 min(P<0.05)。机械复苏组初始5 min、终末5 min PSV平均值及按压频率达标率分别高于徒手复苏组初始5 min、终末5 min,差异有统计学意义(P<0.01),机械复苏组按压达标率为100%。结论机械复苏较徒手心肺复苏可获得较好的颈动脉血流灌注,超声技术对按压频率及颈动脉的血流量可实时反馈,可作为一种无创CPR质量控制的方法。Objective To explore the characteristics and monitoring effect of blood flow ultrasonography in carotid artery during unarmed and mechanical cardiopulmonary resuscitation.Methods A total of 28 patients with cardiac arrest admitted into our department of emergency from November 2015 to November 2016 were selected as research objects.They were evenly divided into unarmed cardiopulmonary resuscitation(CPR) group(n=14) and mechanical CPR group(n =14) in random.In the unarmed CPR group,unarmed chest compression for 30 minutes was used,while in the mechanical CPR group,30-minute mechanical chest compression was adopted.The average of peak systolic velocity(PSV)in carotid artery and frequency of compression in each minute during the initial 5 minutes and the terminal 5 minutes were measured by Ultrasound Doppler technique.Results In the unarmed CPR group,the average PSV value in carotid artery during the initial 5 minutes was higher than that of the terminal 5 minutes with a statistical difference(P〈0.01).The achieved rate of compression frequency was also higher than that in the terminal 5 minutes(P〈0.05).In the mechanical CPR group,the average PSV values in the initial 5 minutes,terminal 5 minutes,and achieved rate of compression frequency were all higher than those in the unarmed CPR group with statistical differences(P〈0.01).The achieved rate of compression in the mechanical CPR was 100%.Conclusion Compared with unarmed CPR,the mechanical CPR can obtain a better carotid artery perfusion.Ultrasonography can realize the real-time feedback of compression frequency and blood flow in carotid artery,and is used as a non-invasive CPR quality control.
