腹腔镜胃癌根治术治疗进展期胃癌的临床效果分析

摘　　要：目的探讨腹腔镜胃癌根治术治疗进展期胃癌的临床疗效。方法选取行腹腔镜胃癌根治术的进展期胃癌患者50例,分析其临床疗效。结果 50例患者均成功完成腹腔镜辅助胃癌根治术,全胃切除、近端胃切除、远端胃切除手术时间分别为(305±45)、(270±380)、(210±40)min,出血量分别为(260±40)、(230±30)、(110±40)mL,清扫淋巴结数目分别为(21.3±2.7)、(16.9±5.2)、(15.5±3.7)枚。手术后患者神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原19-9(CA19-9)、角蛋白21-1片段(CYFRA21-1)、高敏C反应蛋白(hs-CRP)水平与手术前相比均显著改善(P<0.05)。结论腹腔镜胃癌根治术能明显改善血清肿瘤标志物和hs-CRP水平。Objective To explore the clinical efficacy of laparoscopic radical gastrectomy in treatment of patients with advanced gastric cancer. Methods A total of 50 patients with advanced gastric cancer were selected and treated with laparoscopic radical gastrectomy, and efficacy was ana- lyzed. Results All the patients completed surgery successfully, the average operation time was （305 ± 45 ）, （270 ± 380）, （210 ± 40 ） rain in patients with total gastrectomy, proximal gastrectomy, dis- tal gastrectomy, the average blood loss volume was （305 ±45）, （270 ±380） , （210 ±40） min, and the average number of lymph nodes dissected was （21.3 ±2.7） , （ 16.9 ±5.2） , （ 15.5 ± 3.7） re- spectively. The levels of CEA, NSE, CA125, CA19-9, CYFRA21-1 and hs-CRP after operation were significantly lower that those before operation （ P 〈 0. 05 ）. Conclusion Laparoscopic radical gas- trectomy can significantly improve the levels of hs-CRP and serum tumor markers in the patients with advanced gastric cancer.
