宫颈癌根治性放疗/放化疗的疗效及预后相关因素分析

摘　　要：目的分析宫颈癌根治性放疗/放化疗的疗效及预后相关因素。方法回顾性分析在宁德市医院行根治性放疗/放化疗的79例宫颈癌患者的临床资料及临床疗效、生存时间。结果治疗结束后3个月疗效评价为CR的达75.9%,至随访结束为止全组患者未达中位生存期,全组1年生存率98.6%,2年生存率89.3%,3年生存率89.3%,4年生存率78.2%,平均生存时间为48.7个月。预后相关因素分析提示FIGO分期越晚,近期疗效达到CR的概率越低,而达到疗效评价CR的患者生存期比疗效评价为PR的生存期长,肿瘤大体类型为菜花型的生存期较其他类型的生存期长,差异均有统计学意义(P<0.05)。结论宫颈癌行根治性放疗/放化疗可获良好疗效,肿瘤FIGO分期对治疗结束后3个月时的疗效评价有显著影响,而肿瘤大体类型及治疗结束后3个月时的疗效评价是影响患者生存期的独立影响因素。Objective To analyze the curative effect and prognosis-related factors of radical radiation and chemoradiotherapy for cervical cancer.Methods The clinical data, clinical effects and survival time of 79 cases with cervical cancer treated by radical radiotherapy or chemoradiotherapy in Ningde Hospital were retrospectively analyzed. Results The CR rate of therapeutic evaluation after 3 months reached 75.9%.To the end of the follow-up visits, all sufferers failed to reach the median survival.The 1-year survival rate was 98.6% ,the 2-year survival rate was 89.3% ,the 3-year survival rate was 89.3%, and the 4-year survival rate was 78.2%.The average survival time was 48.7 months. Prognosis-related factors analysis showed the later FIGO stage, the lower possibility of CR for short-term effects.The sufferers with therapeutic evaluation of CR had longer survival time than those with PR.The sufferers with tumor type of cauliflower tended to live longer than those with other tumor types, and the differences had statistical significance （P〈 0.05）.Conclusion Radical radiotherapy or chemoradiotherapy for cervical cancer has good curative effects, tumor FIGO stages shows significant influence on the curative effects of 3 months after treatment.Tumor types and therapeutic evaluation of 3 months after treatment are independent influence factors for the sufferers＇ survival time.
