摘 要:目的探讨应用CT测定前列腺周围脂肪组织与前列腺癌危险分级的相关性。方法选取2012年6月至2016年5月于天津医科大学第二医院住院治疗的局限期前列腺癌患者67例,分为低危组、中危组和高危组,所有患者行血清学检查和盆腔CT检查,在CT层面勾画感兴趣区并测量皮下脂肪厚度(SFT,cm)、前列腺周围脂肪面积(PFA,cm2)及前列腺周围脂肪密度(PFD,%),计算每例患者体重指数,统计各肥胖指标与前列腺癌危险分级的相关性。结果三组的前列腺体积、初诊血清前列腺特异性抗原(PSA)、PFA及PFD与前列腺癌危险分级存在统计学相关性。结论应用CT测定前列腺周围脂肪组织可作为一种快速、有效的检测手段对前列腺癌危险分级进行预测。Objective To measure periprestatic fat on computer tomography (CT) and examine if it correlates with high- risk prostate cancer. Methods We retrospectively analysed CT scans of 67 patients with local prostate cancer from June 2012 to May 2016 in The Second Hospital of Tianiin Medical University. Patients were stratified into three groups: low-grade, intermediate-grade and high- grade of prostate cancer risk. Subcutaneous fat thickness (SFT, cm), periprostatic fat area (PFA, cm^2) and periprostatic fat density (PFD, %) were performed on the CT scan. Associations between the three group and BMI, SFT, PFA and PFA were examined. Results Logistic analysis revealed a significant association between prostate volume (P=0.002), initial PSA (P=0.000), PFA (P=0.008) and PFD (P=0.005). Conclusion adipose measured by CT is a rapid and effective method in prediction of prostate cancer aggressiveness.
