摘　　要：目的观察胰岛素样生长因子结合蛋白(insulin-like growth factor binding protein,IGFBP)在早期与进展期胃癌组织中的表达差异,分析IGFBP-3表达与进展期胃癌患者的复发转移和中位生存时间的关系。方法利用Western blot和免疫组化的方法检测IGFBP-3在早期与进展期胃癌中的表达。并用Log-rank检验法统计分析IGFBP-3与患者复发转移和中位生存时间的相关性。结果 Western blot结果显示,IGFBP-3蛋白在进展期胃癌患者血清中的表达(0.709±0.120)低于早期胃癌(0.818±0.038)和正常对照组(0.871±0.037)(P<0.05),在早期胃癌与正常对照血清中的表达差异无统计学意义(P>0.05)。免疫组化结果显示,IGFBP-3在进展期胃癌组中的表达(0.171±0.009)明显低于正常对照组(0.312±0.081)和早期胃癌组(0.309±0.031)(P<0.05)。至随访截止日期,进展期25例根治术后胃癌患者中有10例发生了复发转移,复发转移组中IGFBP-3蛋白表达水平(0.162±0.010)明显低于无复发转移组(0.171±0.006)(P<0.05)。Log-rank检验法统计分析IGFBP-3阴性表达患者中位生存时间明显小于阳性表达患者(P<0.05)。结论 IGFBP-3在进展期胃癌组织和血清中的表达均下调,阴性表达者复发转移率高,生存时间缩短。Objective To observe the expression difference of IGFBP-3 on early and advanced gastric cancer and to explore the relation between IGFBP-3 and survival rate of advanced gastric cancer. Methods The expression difference of IGFBP-3 on early and advanced gastric cancer was measured by Western blot and immunohistochemistry, the relation between IGFBP-3 and survival development of advanced gastric cancer was analyzed with Log-rank method. Results Western blot analysis showed that the expression of IGFBP-3 protein in serum of patients with advanced gastric cancer（0.7092 ± 0.1195 ） was significantly lower than those in patients with early gastric cancer （0.8183 ±0.0378） and normal control （0.8711± 0.0374） （P〈0.05）. However the expression difference between early gastric cancer and normal control serum had no significant. Immunohistochemistry analysis showed that the expression of IGFBP-3 in advanced gastric cancer （0.1707 ± 0.0089） was significantly lower than that in normal gastric mucosa （0.3121± 0.0814） and early gastric tissue （0.3093 ± 0.0314） （P〈0.05）. Conclusion IGFBP-3 protein was down-regulated in both serum and tissues of advanced gastric cancer patients. Moreover, recurrence rate of IGFBP-3 negative expression is increased while survival time of patients with advanced gastirc cancer is shortened.
