摘　　要：目的探究乌司他丁联合奥曲肽治疗老年急性重症胰腺炎患者的机制。方法 180例老年急性重症胰腺炎患者,按随机数字表法分为观察组和对照组各90例,对照组给予常规治疗和乌司他丁治疗,观察组在常规治疗基础上给予乌司他丁和奥曲肽治疗,治疗前和治疗后5 d检测两组免疫细胞、炎症因子和血液流变学变化情况。结果两组治疗前免疫细胞、炎症因子和血液流变学指标比较差异无统计学意义(P>0.05);治疗后炎症因子白细胞介素(IL)-6、IL-8、肿瘤坏死因子(TNF)-α、C反应蛋白(CRP),血液流变学指标(高切全血比黏度、血浆比黏度、血小板黏附率、血栓长度)均显著降低,且观察组低于对照组,免疫细胞CD3^+、CD4^+、CD4^+/CD8^+均显著升高,且观察组显著高于对照组,CD8^+均显著降低且观察组低于对照组(均P<0.05)。结论乌司他丁联合奥曲肽治疗能够改善老年急性重症胰腺炎免疫细胞、炎症因子和血液流变学水平,为急性重症胰腺炎老年患者的临床治疗提供帮助。Objective To observe the mechanism of ulinastatin combined with octreotide for senile acute pancreatitis treatment and offer help for senile acute pancreatitis treatment. Methods A total of 180 senile patients with acute panereatitis were selected and randomly divided into observation and control groups. The senile patients in control group were treated with ulinastatin and the patients in observation group were treated with ulinastatin and octreotide. Inflammatory, immune cell and hemorheology were compared before and after five days therapy. Results Before treatment, inflammatory, immune cell and hemorheology of two groups had no statistically significant differences. The levels of CD8^＋ and hemorheology of two groups were decreased significantly than those before treatment （P 〈 0.05 ）, immune cell （CD3^＋, CD4^＋, CD4^＋/CD8^＋） of two groups were increased significantly than those before treatment （P〈0.05） ; CD8^＋ and hemorheology of observation groups were decreased significantly than those of control group after treatment （P〈0.05） , immune cell （ CD3^＋、 CD4^＋、CD4^＋/ CD8^＋ ） of observation groups were increased significantly than those of control group after treatment （ P〈0.05 ） ; Conclusions Ulinastatin combined with octreotide could perfect effectively inflammatory, immune cell and hemorheology on chile acute panereatitis, it has important clinical significance for acute pancreatitis.
