摘 要:微血管侵犯(MVI)被认为是肝细胞癌术后复发的高危因素,其影响患者预后。而早期预测MVI,对于术前选择肝移植受体、手术方案的制订(解剖性肝切除还是非解剖性肝切除)、术后辅助治疗、判断预后具有重要的指导意义。随着对肝细胞癌MVI患者脱-γ-羧基凝血酶原、甲胎蛋白、甲胎蛋白异质体3、γ谷氨酰转肽酶、微RNA等血清学指标的深入研究及其影像学特点的观察,今后可使用微RNA等血清学指标和典型影像学表现早期预测肝细胞癌MVI。Microvascular invasion(MVI) is considered as a high risk factor for postoperative hepatocellular carcinoma recurrence, which leads to poor prognosis. Early prediction of MVI has a great directive significance to choose the transplan- tation candidates, to decide surgical planning( do anatomical or non-anatomical hepatectomy), to enlarge the hepatic tissue of resection as far as possible, to select postoperative adjuvant therapy and to predict the prognosis. The research of serum molecular biomarkers ( including des-γ-carboxy-prothrombin, alpha-fetoprotein, alpha-fetoprotein lens culinaris agglutinin 3, γ-glutamyl transpepfidase, microRNA etc) is advancing and the imaging features in patients with hepatocellular carcinoma MVI are being discovered continuously. Early prediction of MVI probably will depend on serum microRNA biomarkers and typical imaging features in the future.
