人胃腺癌p38MAPK信号通路表达规律与多药耐药相关因子、病理情况及化疗后预后的相关性研究

摘　　要：目的:探讨人胃腺癌患者组织中p38MAPK表达情况与多药耐药因子P170、GST-π以及临床病理情况、相同化学治疗后患者生存期之间的关系及意义。方法:随机选取大连医科大学附属第一医院2007年至2012年胃癌根治术后且病理类型为胃腺癌,经方案大体一致的系统化学治疗、随诊资料完整的73例患者。应用免疫组化方法对病理标本进行检测,收集p38MAPK以及已知多药耐药因子P170和GST-π的表达情况,结合患者的年龄、性别、肿瘤位置、肿瘤分化程度、肿瘤分期等病理情况和生存期等预后资料,总结后应用SPSS软件对数据进行统计学分析。结果:入选人胃腺癌病例中p38MAPK总阳性表达率为72.6%,但与患者年龄、性别、肿瘤位置、肿瘤分化程度、肿瘤分期无相关性(P>0.05);p38MAPK阳性表达率与P170及GST-π率阳性表达率均呈负相关(P<0.05);经过相同化学治疗后p38MAPK阳性表达患者中位生存时间为22.9个月,比p38MAPK阴性表达的14.7个月明显延长(P<0.05),同时p38MAPK阳性表达率与肿瘤化学治疗预后生存期存在相关性(P<0.05)。结论:p38MAPK阳性表达率与胃腺癌患者的年龄、性别、肿瘤位置、肿瘤分化程度、肿瘤分期无相关性;但p38MAPK阳性表达率与已知多药耐药因子P170和GST-π阳性表达率呈负相关,p38MAPK阳性高表达是胃腺癌化学治疗预后生存期的有利因素,可能降低化学治疗药物的多药耐药性。Purpose ： To evaluate the expression of p38MAPK in human gastric adenocarcinoma and its correlation with P170, GST-Tr, pathological conditions, and the prognosis. Methods： 73 pathological specimens of gastric adenocarcinoma patients with radical surgery, chemical treatment and complete clinical data are were randomly selected from 2005 to 2012 in The First Affiliated Hospital of Dalian Medical University. The data of age, gender, tumor location, pathological type, TNM stage, survival prognosis and the expression of p38MAPK, P170, GST-π （immunohistochemical method） were collected and analyzed by SPSS. Results： p38MAPK total positive expression rate was 72.6%, There was no statistical significance between p38MAPK and age, gender, tumor location, pathological type, TNM stage （ P 〉 0.05 ） ; The positive expression p38MAPK was negatively correlated with P170, GST-π （P 〈0.05）. The median survival time of p38MAPK positive expression （22.9 months） was longer than that of p38MAPK negative expression （14. 7 months） after chemotherapy （P 〈 0. 05 ）. Conclusion： p38 MAPK positive expression is not correlated with age, gender, tumor location, pathological type, TNM stage ; High p38MAPK positive expression, reducing the degree of multi-drug resistance, is a favorable factor for prognosis of gastric adenocarcinoma chemotherapy.
