摘　　要：目的:分析并比较胸水转移性肺腺癌与胸膜恶性间皮瘤应用沉渣包埋法的与免疫组化法进行鉴别诊断的效果。方法:选择我院住院部自2015年1月~2016年10月经临床病史、影像学资料及病理组织学证实为肿瘤性胸水的患者共计52例作为研究对象,对患者临床资料进行回顾分析。对细胞学涂片法、沉渣包埋法及免疫组化法下对胸水转移性肺腺癌与胸膜恶性间皮瘤的诊断结果进行对比,分析不同诊断方法下诊断结果差异。结果:胸水转移性肺腺癌患者CEA、TTF-1、Galectin-3阳性检出率分别为95.24%、83.33%、97.62%,均显著高于胸膜恶性间皮瘤患者,差异均有统计学意义(P<0.05)。结论:沉渣包埋法与免疫组化法对胸水转移性肺腺癌及胸膜恶性间皮瘤的病理诊断分型效果较常规细胞涂片法更好。Objective： To analyze and compare the differential diagnosis of pleural malignant pleural effusion with metanocarcinoma and pleural malignant mesothelioma using sediment embedding method and immunohistochemical method. Metliods ： Fifty-two patients with malignant pleural effusion were enrolled in our hospital from January 2015 to October 2016,with clinical history,imaging data, and histopathology. The patients were clinically diagnosed Data were analyzed retrospectively. The results of cytological smear, sediment embedding and immunohistochemistry were used to compare the diagnostic results of pleural metastatic adenmesothelioma under different diagnostic methods. Results： The positive rates of CEA, TTF-1 and Galectin-3 were 95. 2 4%, 83. 33% and97.62%respectively in patients with metastatic adenocarcinoma of pleural effusion, Mesothelioma patients,the （P 〈 0. 05）. Conclusion ： The method of sediment embedding and immunohistochemistry is more effective in the pathologic diagnosis of metastatic lung adenocarcinoma of pleural effusion and malignant pleural mesothelioma than that of conventional cell smear.
