肠内营养联合生长抑素对胰腺手术患者康复的影响

摘　　要：目的:研究肠内营养联合生长抑素对胰腺手术患者康复的影响。方法:152例胰腺手术患者分为两组,每组76例,给予肠外营养支持为对照组,给予肠内营养支持为观察组,两组均联合生长抑素进行治疗。比较治疗前后两组血糖、血红蛋白、血清白蛋白水平,以及治疗后的氮平衡、肌酐(Cr)、皮质醇(Cor)和血浆胰岛素水平,观察两组恢复情况及并发症发生情况。结果:治疗后,两组血糖、血红蛋白和血清白蛋白水平均较治疗前明显升高(P<0.05),观察组升高程度明显高于对照组(P<0.05)。治疗后,观察组患者的氮平衡、Cr升高指数及Cor水平均明显高于对照组,血浆胰岛素水平明显低于对照组(P<0.05)。观察组术后恢复情况及并发症发生情况均优于对照组(P<0.05)。结论:肠内营养联合生长抑素可加速胰腺手术患者的术后康复,值得临床采用。ObjectiveTo study the influence of enteral nutrition combined with somatostatin on postoperative rehabilitation ofpatients with pancreatic surgery. Methods ： A total of 152 patients with pancreatic surgery were divided into control group treated withparenteral nutrition and observation group with enteral nutrition on the basis of somatostatin ,76 cases for each group. The levels of bloodglucose, hemoglobin and serum albumin before and after treatm ent, and levels of nitrogen balance, creatinine （ Cr） , cortisol （C or） andplasma insulin after treatment were compared between two groups. The rehabilitation conditions and incidence of complications were observedin both groups. Results ： After treatm ent, levels of blood glucose, hemoglobin and serum albumin increased in both groups thantreatment before （ P 〈 0 .0 5 ） ,which were markedly higher in observation group than those in control group （ P 〈 0. 05 ） . After treatment, observation group was notably higher in the levels of nitrogen balance, Cr and Cor,but was prominently lower in the level of plasmainsulin than control group （ P 〈 0. 05 ） . M eanwhile, observation group was also significantly better than control group in the postoperativerehabilitation conditions and occurrence rates of complications （ P 〈 0 . 0 5 ）. Conclusion ： Enteral nutrition combined with somatostatincan promote the postoperative rehabilitation of patients with pancreatic surgery, thus deserving to be widely used in clinic.
