摘　　要：目的探讨翼状胬肉切除联合自体角膜缘结膜移植术治疗翼状胬肉对患者术后干眼及泪膜功能的影响。方法将78例(78眼)原发性静止期翼状胬肉患者按随机数字表法分为观察组(51例51眼)和对照组(27例27眼),对照组行单纯翼状胬肉切除术,观察组行翼状胬肉切除联合自体角膜缘结膜移植术。观察2组术后1周、1个月、3个月患眼角膜上皮修复情况及泪膜破裂时间(BUT)、泪液分泌试验(SchirmerⅠ试验)结果、角膜荧光染色(FL)评分。结果术后1周观察组角膜上皮修复0级、1级所占比例明显高于对照组,2级所占比例明显低于对照组(P<0.05);术后1、3个月2组患者角膜上皮修复情况比较差异无统计学意义(P>0.05)。术后1周、1个月、3个月观察组BUT均明显高于对照组(P<0.05),而SchirmerⅠ试验结果及FL评分2组对比差异均无统计学意义(P>0.05)。结论翼状胬肉切除联合自体角膜缘结膜移植术后患者干眼症发生率较单纯翼状胬肉切除术低,角膜上皮修复较好,泪膜破裂时间较长。Objective To explore the effects of pterygium excision combined with limbal con- juctival autograft on postoperative dry eye and tear film function. Methods A total of 78 patients with primary stationary phase of pterygium （78 eyes） were randomly treated with pterygium exci-sion alone （observation group, 51 patients, 51 eyes） or in combination with limbal conjuctival au-tograft （control group, 27 patients, 27 eyes）. The corneal epithelium repair,tear break-up time （BUT）,Schimer I test and corneal fluorescence staining （F L ） scores were measured 1 w e ek, 1 month and 3 months after operation. Results Compared with control group, limbal conjuctival au-tograft increased the proportion of stage 0 and 1 corneal epithelium repair at 1 week after opera-tion, decreased the proportion of stage 2 corneal epithelium repair at 1 week after operation, and prolonged the BUT at 1 week,l month and 3 months after operation （P〈0 . 05）. There were no significant differences between the two groups in corneal epithelium repair at 1 and 3 months after operation,as well as in Schimer I te s t results and FL scores （P〉0. 05）. Conclusion The combi-nation of pterygium excision and limbal conjuctival autograft is superior to pterygium excision a-lone for repairing corneal epithelium,reducing the incidence of dry eye and prolonging the BUT.
