《伤寒杂病论》郁证特点

摘　　要：仲景所论之郁证为广义的郁证,其包含人体脏腑经络、气血津液、饮食情志等多方面的壅滞、痞塞、蓄积、瘀结、失舒、失畅,为病机的概念。郁证由诸多因素造成,其非独为情志一端,外感六淫之邪气,饮食劳倦之所伤,脏腑气血津液之失常,或者失治误治等皆可导致郁证。《伤寒杂病论》描述的是一种滞而不通,结而不散的病理状态,其以气之升降出入失常为基础病机,气郁、血郁、水郁、火郁均蕴含气机升降出入失常之内涵,气郁为诸郁之先,亦为诸郁之结果。仲景对郁证之症状描述,既包括病机的内涵,亦包含有症状之内涵,对郁证病位、性质和症状的诊断和鉴别诊断具有重要意义,其既有对《黄帝内经》之发挥,又有对各种临床所见郁证之总结,主要表现为满、痹、着、结、痞等。According to Zhang Zhongjing, depression syndrome is a mechanism concept in a broad sense, including the stagna- tion, stuffiness, accumulation, blood stasis and unsmootbness of the viscera, meridians and eollaterMs, qi, blood and body flu- id, diet and emotions. Depression syndrome is caused by multiple factors except for emotional factor. Exogenous six climatic pathogens, improper diet, overstrain, abnormalities of visceral qi, blood and body fluid, failure of treatment and misdiagnosis can also lead to depression syndrome. The depression syndrome in Treatise on Cold Damage and Miscellaneous Diseases is a conz cept of pathogenesis, discribing a pathological state of stagnation and accumulation and based on abnormal qi movement. All of qi depression, blood depression, water depression and fire depression include the connotation of disorder of ascending, descending, entering and exiting of qi movement, and qi depression is both the primary, and consequent depression of other depressions. The description of the symptoms of depression syndrome by Zhang Zhongjing not only includes the connotation of pathogenesis but also the connotation of symptoms, having great significance for the differentiation of the disease location, nature as well as the diagnO- sis. It is both the development of The Inner Canon of Huangdi and the summary of various depression syndromes in clinic with main manifestations of fullness, pain, dampness, stagnation and lump, etc.
