调强放疗联合与不联合卡培他滨加顺铂同步化疗治疗中晚期食管癌患者的临床疗效

摘　　要：目的探讨调强放疗联合与不联合卡培他滨加顺铂同步化疗治疗中晚期食管癌患者的临床疗效。方法选取2012年5月至2014年1月成都市第五人民医院收治的60例中晚期食管癌患者作为研究对象,按治疗方式分为观察组(调强放疗联合卡培他滨加顺铂同步化疗)和对照组(单纯调强放疗),比较两组患者的近期疗效与1年、2年生存率及不良反应发生情况。结果观察组患者近期疗效明显高于对照组,差异有统计学意义(P<0.05);观察组患者治疗后1年生存率明显高于对照组,差异有统计学意义(P<0.05);观察组患者恶心、呕吐及白细胞下降、手足综合征、血小板减少发生率均明显高于对照组,差异均有统计学意义(均P<0.05)。结论在中晚期食管癌患者治疗过程中,调强放疗联合卡培他滨加顺铂行同步化疗能够使患者获得更佳疗效,并提高1年生存率,不良反应可耐受。Objective To investigate the clinical efficacy of intensity modulated radiotherapy combined with capecitabine and cisplatin in the treatment of advanced esophageal cancer.Methods 60 patients with advanced esophageal cancer were selected from May 2012 to January 2014 of the fifth Chengdu Municipal People’s Hospital as the research object,the patients were divided into observation group（intensity-modulated radiotherapy combined with capecitabine plus cisplatin chemotherapy） and control group（simple IMRT）,the occurrence of relatively recent curative effect of two groups of patients with 1 and 2 years survival rate and adverse reaction.Results The curative effect of patients was significantly higher than the control group,the difference was statistically significant（P〈0.05）;patients in the observation group after treatment,1 year survival rate was significantly higher than the control group,the difference was statistically significant（P〈0.05）;the patients in the observation group were nausea,vomiting and leukopenia,hand foot syndrome,the incidence of thrombocytopenia were significantly higher than the control group,the differences were statistically significant（P〈0.05）.Conclusion Intensity modulated radiation therapy with capecitabine and cisplatin concurrent chemotherapy for patients with advanced esophageal cancer improved the short-term efficacy and 1 year survival rate,while the side effects were tolerable.
