自血疗法治疗溃疡性直肠炎30例临床观察

摘　　要：为了观察自血疗法治疗溃疡性直肠炎的临床疗效，将2012年6月至2016年12月，广西壮族自治区柳州市人民医院中医肛肠科诊治的轻中度活动期溃疡性直肠炎患者90例，随机分为治疗组（自血疗法）和对照1组（美沙拉秦栓）、对照2组（康复新液），每组各30例。治疗组选取双侧手三里、足三里穴位注射自体静脉血1mL，每7d治疗1次，每3次为1个疗程，共治疗3个疗程。对照1组采用美沙拉秦栓早、晚各1枚纳肛，对照2组采用康复新液50mL每晚保留灌肠1次，2组均7d为1个疗程，共治疗9个疗程。结果显示，在改善患者的临床症状，疾病活动指数（Sutherland DAI）评分及血清IL-8、IL-10水平方面，治疗组优于对照1、2组。3组患者治疗后6个月DAI评分，治疗组（1．6±2．1）分，低于对照1、2组（5．6±1．4）、（6．5±1．3）分（P〈0．05）。治疗后治疗组患者血清IL-8水平下降，血清IL-10水平提高，与对照1、2组比较（P〈0．05）。治疗组总有效率86．7％（26／30）明显优于对照1组63．3％（19／30）、对照2组60．0％（18／30）。差异均有统计学意义（P均〈0．01）。结果表明，自血疗法能够调节患者血清IL-8、IL-10水平，治疗溃疡性直肠炎疗效确切，未见不良反应，方法安全可靠。This study was to observe the clinical effect of autohemotherapy for ulcerative proetitis（UP）, randomly divided 90 cases of UP（being at active stage of mild and middle degree） who admitted in author＇s department from Jun. 2012 to Dee. 2016 into treament group （autohemotherapy） and No. 1 control group （mesalazine suppository）,No.2 control one（Kangfu Xinye）,30 cases for each;in treatment group at Shou- sanli（LI10） point and Zusanli（ST36） point of bilateral hands and feet. performed autohemotherapy,injected antologous venous blood lmL 1 time/7 d×3 as a medical course,totaled 3 course;No.1 control group adminitered suppository, 1 grain each morning and night rectal administration;No.2 control one adopted re- tention enema with Kangfu Xin solution 50 mL 1 time each night No. 1 and No. 2 received all 9 medical course（1 course/7 d）.As results,after treatment in improvement of clinical symptoms, in score rating of Sutherland DAI,and in level of serum IL-8 and IL-10 treatment group was all superior to other groups; 6 months after treatment DAI score rating of treatment group was significantly lower than that of No.1 and No.2 groups （1.6±2.1 points VS 5.6±1.4 and 6.5±1.3 points, P 〈0.05） ;after treatment in treatment group serum IL-8 level decreased,IL-10 level uplifted,there was significant difference compared with No.1, No.2 group （all, P 〈0.05）;total effective rate of treatment group was significantly higher compared with No.1 and No.2 group （86.7%,26/30 vs 63.3%,19/30 and 60.0% ,18/30） ,there was statistical significance in difference（all, P 〈0.01）.Results show that autohemotherapy can regulated patient＇s serum IL-8 and IL- 10 level, has determined effect for treating UP, and without any adverse reaction, and its application method is safe and reliable.
