摘 要:目的:建立大鼠Walker-256乳腺癌细胞骨癌痛模型,探讨乳腺术后骨转方对骨癌痛的镇痛作用。方法:SD大鼠54只,按体重随机分成6组,每组9只,于大鼠左后肢注射Walker-256细胞建立骨癌痛模型,观察各组大鼠造模前后体重、造模后热痛敏阈值的变化、胫骨CT影像、胫骨HE病理染色及破骨计数情况。结果:1造模后第7天乳腺术后骨转方高剂量+唑来膦酸组体重高于模型组(P<0.05);造模后第14天,乳腺术后骨转方高剂量+唑来膦酸组和乳腺术后骨转方低剂量组体重高于模型组(P<0.01)。2乳腺术后骨转方各剂量组在造模后第21天的热痛敏阈值高于模型组(P<0.05),至造模后28 d,乳腺术后骨转方高剂量+唑来膦酸组热痛敏阈值仍高于Model组(P<0.05)。3造模后第28天,乳腺术后骨转方各剂量组可以减轻模型大鼠的骨质破坏以及肿瘤浸润,各组破骨细胞计数低于模型组(P<0.01)。结论:乳腺术后骨转方可以显著提高模型大鼠的热痛敏阈值,减轻热痛敏反应,表现出明显的镇痛作用。其次,本方在一定程度上能够抑制骨癌痛模型大鼠体重的下降、减轻骨质的破坏。乳腺术后骨转方联合唑来膦酸等够早期干预模型大鼠体重下降的情况,并可以延长减轻热痛敏反应的作用时间。Objective : Set up tibial cancer pain model induced by Walker 256 mammary glad carcinoma cells in rats, to investigate the analgesic effect of Postoperative Breast Guzhuan Decoction. Methods : 54 SD rats were randomly divided into 6 groups according to the weight, each group had 9 rats. Injected Walker 256 mammary gland carcinoma cells into the tibia medullary cavity via intereondylar eminence to prepare the bone cancer pain model. Observed the weight, the PWTL before and after model made, tihial CT images, tibial pathological staining and osteoclast count. Results: (1)The weight of Z+H group was higher than model group on the 7th day after model made ( P〈0.05 ). The weight of Z+H group and L group was higher than model group on the 14th day after model made ( P〈0.01 ). (2)The PWTL of each dosage group of Postoperative Breast Guzhuan Decoction was longer than model group ( P〈0.05 ), the PWTL of Z+H group was still higher than model group on the 28th day after model made ( P〈0.05 ). (3)Each dosage group of Postoperative Breast Cuzhuan Decoction could restrain the bony destruction and tumor invasion in the model rats, the osteoclast count of each group was lower than the model group ( P〈0.01 ). Conclusion : Postoperative Breast Guzhuan Decoction could raise the PWLT in order to relieve thermal pain sensitivity which showed a obvious analgesic effect. Secondly, Postoperative Breast Guzhuan Decoction also could improve the weight loss of rats after model were made, alleviate the rat bony destruction in a degree. The combination of Postoperative Breast Guzhuan Decoction and zoledronic acid could early relieve model rats' weight loss, and could prolong the restrain time of heat pain response.
