新疆某三级医院胃癌超长住院多因素分析

摘　　要：目的分析影响重大疾病住院天数的主要因素,提出缩短超长住院天数的方法。方法从新疆某三级医院信息系统调取2015年下半年胃癌出院患者信息,收集1 588例胃癌患者住院临床信息,利用Excel表格功能以及多因素Logistic回归分析,了解胃癌患者超长住院天数的主要影响因素。结果 Logistic回归模型显示,胃癌患者的肿瘤分期、治疗方案、会诊情况、转科情况、抢救情况、手术等级、切口等级以及是否有消化道疾病对病人的住院天数的影响有统计学意义(P<0.05)。结论疾病进展程度、诊疗方案、会诊、转科、手术等级、切口大小以及是否有伴随性疾病等为影响胃癌超长住院天数主要的因素;对病人进行及时、合理的诊疗、分科、会诊以及简化程序是缩短住院天数的有效措施;只有综合考虑所有影响因素,有重点地控制主要因素,才能最大程度地缩短住院天数。Objective The aim of this study was to find the reasons for influencing the length of hospital stay and put forward the method to shorten the hospitalization days through the comprehensive analysis of 1588 patients with gastric cancer in certain hospital in Xinjiang. Methods The discharged gastric cancer patients＇ information in the second half of 2015 were extracted from the hospital information retrieval system and analyzed using non-conditional logistic regression model and excel function by SPSS18.0 for the influencing factors of hospital overstay. Results Logistic regression analysis showed that the tumor staging, treatment, consultation, transfer, emergency treatment, operation level, incision level and accompanying digestive tract diseases were statistically significant for patients with gastric cancer （P 〈 0.05）. Conclusion 1.The long hospitalization was mainly affected by the severity of the disease itself, hospital treatment, consultation, transfer, medical operation level and whether the patients were suffering from concomitant diseases. 2. The timely and rational treatment, division, consultation and simplified procedure toward the patients＇ cases were the effective ways to shorten the days of hospitalization. 3. The hospitalization days could be diminished efficiently only if all the influencing factors were taken into consideration and the main influencing factors were controlled sufficiently.
