肠内生态免疫营养在胃肠道肿瘤患者围手术期中的应用

摘　　要：目的研究分析肠内生态免疫营养在胃肠道肿瘤患者围手术期中的临床应用效果。方法 90例行外科手术治疗的胃肠道肿瘤患者作为研究对象,随机分成对照组及观察组,各45例。对照组围手术期给予传统常规的肠外营养支持,观察组围手术期给予肠内生态免疫营养干预措施。比较两组患者手术前、后免疫球蛋白(Ig)指标(IgA、IgG、IgM)、T淋巴细胞亚群(CD4+、CD8+、CD4+/CD8+)以及不良反应。结果手术前,两组患者的IgA、IgG、IgM免疫球蛋白水平及CD4+、CD8+、CD4+/CD8+T淋巴细胞亚群水平比较,差异均无统计学意义(P>0.05);手术后均较手术前改善,且观察组改善程度优于对照组,差异均具有统计学意义(P<0.05)。观察组术后不良反应发生率为8.89%,低于对照组的35.56%,差异具有统计学意义(χ~2=9.257,P<0.05)。结论围手术期给予肠内生态免疫营养干预能够明显提高胃肠道肿瘤患者的免疫能力,降低术后不良反应发生率,临床应用效果良好,值得应用推广。Objective To study and analyze the clinical application effect of intestinal ecology immune nutrition in gastroenteric tumor patients during perioperative period. Methods A total of 90 gastroenteric tumor patients treated with surgical operation as study subjects were randomly divided into control group and observation group, with 45 cases in each group. The control group received traditional and conventional parenteral nutrition support, and the observation group received intestinal ecology immune nutrition intervention measure. Comparison were made on immunoglobulin（Ig） indexes（IgA, IgG, IgM）, T lymphocyte subsets（CD4＋, CD8＋, CD4＋/CD8＋） before and after operation and adverse reactions in two groups. Results Before operation, both groups had no statistically significant difference in immunoglobulin level of IgA, IgG, IgM and T lymphocyte subsets of CD4＋, CD8＋, CD4＋/CD8＋（P〈0.05）. Both groups had better indexes after operation than before operation, and the observation group had better improvement than the control group. Their difference had statistical significance（P〈0.05）. The observation group had lower incidence of adverse reactions as 8.89% than 35.56% in the control group, and the difference had statistical significance（χ^2=9.257, P〈0.05）. Conclusion Intestinal ecology immune nutrition intervention can clearly increase immune ability of gastroenteric tumor patients and decrease incidence of postoperative adverse reactions with good clinical application effect. So it is worthy of application and promotion.
