摘　　要：目的探讨甲状腺癌颈部淋巴结转移分布的规律及术前超声的诊断价值。方法回顾413例病理证实甲状腺癌患者资料,术前行超声检查,按颈部淋巴结分区对淋巴结转移情况进行评估,记录可疑阳性淋巴结分布,以术后病理诊断为金标准,计算超声对于甲状腺癌淋巴结转移的诊断符合率。结果超声对淋巴结转移的诊断符合率为84.26%,灵敏度为82.35%,特异度为85.03%。甲状腺癌颈部淋巴结转移多见于中央区,显著高于侧颈淋巴结转移,差异具有统计学意义(P<0.01)。术前超声对甲状腺癌颈部淋巴结转移的诊断与病理有较髙的符合率。结论术前超声检查能对甲状腺癌颈部淋巴结转移作出准确评估,对手术清扫范围的选择具有一定的指导意义。Objective To investigate the distribution law of thyroid cancer cervical lymph node metastases and preoperative ultrasonography diagnostic value. Methods To review 413 cases of pathologically proven thyroid cancer patient data, preoperative ultrasound, according to the partition to assess lymph node metastasis to the cervical lymph nodes, record suspicious positive lymph nodes distribution, the postoperative pathological diagnosis as the gold standard, calculate the diagnostic accordance rate of ultrasound for the thyroid cancer lymph node metastasis. Results The diagnostic accordance rate of ultrasound for lymph node metastasis was 84,26%, the sensitivity was 82.35%, 85,03%. Thyroid carcinoma of cervical lymph node metastasis in central region, more is significantly higher than the lateral transfer analysis, which difference was statistically significant （P 〈 0.01）. Preoperative ultrasonography in the diagnosis of thyroid carcinoma of cervical lymph node metastasis and pathological has a high coincidence rate. Conclusion Preoperative ultrasound can make an accurate assessment of thyroid cancer cervical lymph node metastases, the selection of surgical cleaning scope has a certain guiding significance.
