摘 要:目的探讨乳腺浸润性癌MRI表现与生物因子雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER2)、肿瘤增殖抗原Ki-67、肿瘤抑制蛋白p53表达的相关性及临床意义。方法回顾性分析69例乳腺浸润性癌患者的MRI表现及生物因子ER、PR、HER2、Ki-67、p53的表达情况,采用Spearman相关分析和分类回归树(CART)算法分析MRI表现与各生物因子表达的相关性。结果 HER2表达与淋巴结转移呈正相关(r=0.299,P﹤0.05),p53表达与病变表现为肿块呈负相关(r=-0.261,P﹤0.05);肿块分叶征象与Ki-67(r=0.472,P﹤0.01)、p53(r=0.25,P﹤0.05)阳性表达呈正相关。根据MRI表现分析各生物因子表达的CART决策树,分类准确度依次为:Ki-67(0.797)﹥ER(0.754)﹥PR(0.725)﹥HER2(0.478)﹥p53(0.464)。结论乳腺浸润性癌的MRI表现与生物因子ER、PR、HER2、Ki-67、p53的表达有一定的相关性,可作为乳腺癌的重要诊断指标。Objective To investigate the correlation between the MRI manifestations and expression of estrogen re- ceptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2), Ki-67, p53 in invasive breast cancer. Method The MRI features of 69 cases of pathologically confirmed invasive breast cancer, and the expres- sion of ER, PR, HER2, Ki-67, p53 were retrospectively analyzed, Spearman' s rank correlation and Classification and Re- gression Tree (CART) were used to determine the correlations. Result Lymph node metastasis was positively correlated with ttER2 value (r=0.299, P 〈 0.05), while mass lesion was negatively correlated with p53 expression (1=-0.261, P 〈 0.05); lobulation sign was positively correlated with Ki-67 (r=0.472, P〈0.01) and p53 (r=0.25, P〈0.05). CART decision trees were generated as per MRI features, with classification accuracy as: Ki-67 (0.797) 〉 ER (0.754) 〉 PR (0.725) 〉 HER2 (0.478) 〉 p53 (0.464). Conclusion MRI manifestations are partially associated with the expression of ER, PR, HER2, Ki-67, p53 in invasive breast cancer, enabling them to be important diagnostic indicators.
