下颌骨正中矢状面分析在面部偏斜诊疗中的补充应用

摘　　要：目的采用下颌正中矢状面分析面部偏斜患者下颌骨轮廓和位置的对称性,观察其在临床诊疗中的应用效果。方法选取面部偏斜患者45例,根据三维测量结果将患者分为下颌骨轮廓对称组和非对称组;以下颌骨正中矢状面作为参考平面,并利用镜像技术分析下颌骨位置及其轮廓的对称性,制定手术方案;通过Pearson相关性检验分析Me-MSP与下颌左右侧轮廓测量项目间差值的相关性,配对t检验比较下颌骨轮廓非对称组在治疗前后的差异。结果轮廓对称组与非对称组相比,Go-_(lat)-MSP、Me-MSP、MPP3-MSP、MPP7-MSP、MLP3-OP、MLP7-OP、Go-_(inf)-OP差异具有统计学意义(P<0.05);非对称组治疗后与治疗前相比,除Go-_(lat)-MSP外,Me-MSP、MPP3-MSP、MPP7-MSP、MLP3-OP、MLP7-OP、Go-_(inf)-OP差异均有统计学意义(P<0.05);Me-MSP与双侧MPP3-MSP差值呈正相关关系(r=0.684,P=0.001),而与其他的变量差值无相关关系。结论该方法能定量分析下颌骨轮廓和位置的不对称畸形,对于颌部整形有良好的临床指导作用,患者满意度较高。Objective To use the mandibular mid-sagittal plane to analyze the symmetry of contours and position of the mandible in facial asymmetry patients and to observe its effect in clinical diagnosis and treatment.Methods This study consisted of 45 patients with facial asymmetry.All patients were classified into 2 groups according to three--dimensional measurement results：for the mandibular contour symmetric group and the mandibular contour asymmetric group.The mandibular mid-sagittal plane was used as a reference plane,and mirror technology was employed to analyze the symmetry of contours and the position of the mandible.From this data,the operation plan was created.The correlation of differences between Me-MSP and the measured points of the mandibular contour were analyzed by the Pearson correlation test,pre-and post-operation.The paired t-test was used to compare the differences of the mandibular contour asymmetric group before and after treatment.Results The differences between the two groups in Go-lat-MSP,Me-MSP,MPP3-MSP,MPP7-MSP,MLP3-OP,MLP7-OP,Go-inf-OP were statistically significant （P〈0.05）.The difference between the pre-and postoperative Me-MSP,MPP3-MSP,MPP7-MSP,MLP3-OP,MLP7-OP,Go-inf-OP in the mandibular contour asymmetric group was statistically significant （P〈O.001）.There was a positive correlation between Me-MSP and bilateral MPP3-MSP （r=-0.684,P =0.001）,but no correlation with other variables.Conclusion This method can quantitatively analyze the asymmetry deformity of the mandibular contour and position and offers good clinical guidance for contour surgery.The patients＆#39; satisfaction was high and no medical dispute occurred.
