摘 要:目的探究翼状胬肉切除联合带角膜干细胞的自体结膜移植术患者围术期角膜缘部上皮厚度变化规律。方法选取行翼状胬肉切除联合带角膜缘干细胞的自体结膜移植术患者30例(30只眼)为观察组和健康体检者30例(30只眼)为对照组,对比两组研究对象的一般资料、对照组和观察组手术后不同时间点的鼻侧角膜缘上皮厚度、观察组手术后不同时间点的角膜荧光染色缺损面积。结果观察组患者术后1、2、3、5、7天的鼻侧角膜缘内1mm的上皮厚度较对照组薄(P<0.05),观察组患者术后1、2、3、5天鼻侧角膜缘内2mm的上皮厚度较对照组薄(P<0.05),术后7天的鼻侧角膜缘上皮厚度和对照组之间的差异无统计学意义(P>0.05);观察组术后1天的角膜荧光染色缺损面积为(39.29±22.68)mm2,术后2、3、5、7天的角膜荧光染色缺损面积均较前一时间点的角膜荧光染色缺损面积缩小(P<0.05)。结论翼状胬肉切除联合带角膜缘干细胞的自体结膜移植术后角膜上皮缺损的愈合由缺损的周围向中央愈合,而其角膜缘部上皮的厚度则由角膜中央向周围逐渐恢复正常。Objetive To investigate the changes of corneal limbal epithelial thickness in patients with pterygium excision combined with limbal stem cell autograft transplantation. Method 30 patients (30 eyes) treated with pterygium excision combined with limbal stem cell autografl transplantation were selected as the observation group, and 30 Healthy examined people (30 eyes) as control group. General information, the nasal corneal epithelial thickness and the corneal fluorescent staining defect area at different time points after the operation were compared between two groups. Result The epithelial thickness of the l mm in the nasal limbus of 1, 2, 3, 5, 7 days of observation group were thinner than that of control group (P 〈 0.05), the epithelial thickness of the 2mm in the nasal limbus of 1, 2, 3, 5 days of observation group were thinner than that of control group (P 〈 0.05), the epithelial thickness of the 2ram in the nasal limbus in 7 days had no significant difference between two groups (P 〉 0.05); the corneal fluorescence staining defect area of 1 day after operation of observation group was (39.29 + 92.68) mm^2, the corneal fluorescence staining defect area of 2, 3, 5,7 days after operation were smaller than that of the previous time point (P 〈 0.05). Conclusion After pterygium excision combined with limbal stem cell autograft transplantation, the thickness of the corneal epithelium was gradually restored from the center to the periphery.
