单腔和双腔气管插管在胸段食管癌微创McKeown术中的应用比较

摘　　要：目的对比分析胸段食管癌行微创McKeown术中单双腔气管插管的应用效果及对手术效果及并发症的影响。方法回顾性分析2015年3月至2016年9月行微创McKeown术患者的临床资料。结果单腔组肺部并发症发生率(15.45%)低于双腔组(37.33%),平均手术时间和插管时间较双腔组短,喉返神经淋巴结清扫数目较双腔组多,差异均有统计学意义(P<0.05)。结论胸段食管癌患者在行微创McKeown术时,单腔气管插管较双腔支气管插管简单、易行,术后肺部并发症发生率低,同时更有利于喉返神经淋巴结清扫,尤其是左侧喉返神经链淋巴结清扫。Objective To analyze the application effects of single lumen endotracheal tube and double lumen endotracheal tube in McKeown minimally invasive esophagectomy for thoracic esophageal carcinoma and the influences on the operation effects and complications. Methods Clinical data of patients undergoing McKeown minimally invasive esophagectomy from March of 2015 to September of 2016 were retrospective analyzed. Results Compared with double lumen tube group, single lumen tube group have a lower pulmonary complication rate （ 15.45% vs 37.33% ）, the average operation time and average intubation time were shorter, the harvested lymph nodes of recurrent laryngeal nerve were less, and the differences were statistically significant （P 〈 0. 05）. Conclusion In the McKeewn minimally invasive of thoracic esophagea, single lumen tube is simpler and more easily to be operated than the double lumen tube, with lower incidence of postoperative pulmonary complications. At the same time, single lumen endotracheal tube is conducive to cleaning lymph nodes of recurrent laryngeal nerve, especially for the left re- current laryngeal nerve.
