摘 要:目的:探讨翼状胬肉切除术联合口腔黏膜移植术治疗翼状胬肉的临床效果。方法:选取2012年7月至2015年8月广西河池市第一人民医院收治的翼状胬肉患者120例,按随机数字表法分为观察组(60例,78眼)和对照组(60例,75眼),观察组采用翼状胬肉切除术联合口腔黏膜移植术治疗,对照组采用翼状胬肉切除术治疗。比较两组手术时间、术后恢复时间、视觉模拟量表(VAS)评分和复发率。结果:观察组的手术时间明显长于对照组,但角膜上皮愈合时间和移植片愈合时间短于对照组,差异均具有统计学意义(均P<0.05)。术后1d、4d、7d两组患者的VAS评分持续降低,且观察组患者术后4d、7d的VAS评分明显低于对照组,差异具有统计学意义(P<0.05)。观察组术后3个月、6个月、12个月的复发率分别为5.13%、11.54%、16.67%,显著低于对照组的28.00%、42.67%、52.00%,差异均具有统计学意义(均P<0.05)。结论:翼状胬肉切除术联合口腔黏膜移植术治疗原发性翼状胬肉的疗效显著,能够减轻患者术后疼痛,促进角膜愈合,具有复发率低、安全可靠的特点。Objective: To investigate the clinical effect of pterygium excision combined with oral mucosa transplantation in the treatment of pterygium. Methods: 120 patients with pterygium treated in our hospital from July 2012 to August 2013 were selected and randomly divided into an observation group and a control group, with 60 cases in each group. Patients in the observation group received pterygium excision combined with oral mucosa transplantation, while those in the control group received pterygium excision only. The operation time, postoperative recovery time, visual analogue scale (VAS) score and recurrence rate were compared between the two groups. Results: The operative time was higher, while the corneal epithelial healing time and graft healing time were lower than those in the control group (P〈0.05). The VAS scores on day 1, 4 and 7 postoperation in both groups were continuously decreased, but more significantly decreased in the observation group (P〈0.05). After 3, 6 and 12 months of treatment, the recurrence rates were respectively 5.13%, 11.54% and 16. 670/oo in the observation group, and were 28.00 %, 42.67% and 52.00% in the control group. There was significant difference in the recurrence rate between the two groups (P〉0.05). Conclusion: Pterygium excision combined with oral mucosa transplantation in the treatment of primary pterygium was effective. It could relief postoperative pain and promote healing of the cornea, with low recurrence rate, safe and reliable.
